Job summary
An exciting opportunity has arisen for a Healthcare Assistant to join our Primary Care Home, delivering high quality nursing care to the population of Gorleston and surrounding villages. A Primary Care Home (PCH) is a multidisciplinary team working closely with GP's and other community partners to provide an excellent standard of person centered care to adults in their own homes, residential care or clinic settings, supporting patients to self manage whenever possible.
Previous experience of health or social caring for patients in the community would be advantageous.
We will support you to transfer your existing knowledge and develop new clinical skills to become a Community Health Care Assistant. As a Health Care Assistant you will work develop skills in simple wound care, pressure ulcer prevention and care, phlebotomy, and supporting the registered nurses in the delivery of palliative and end of life care. The successful candidate will have NVQ level 3 in Health and Social care or a commitment to completing this qualification.
Main duties of the job
The Health Care Assistant will work with other members of the Primary Care Home multidisciplinary team to provide planned and unplanned community services.
The post holder will be part of a team trained in a range of skills, to deliver clinical interventions for patients with a wide range of health care needs in a variety of community settings.
The role requires the successful applicant to visit patients in their own homes or residential care setting.
You will be requried to lone work, and drive yourself around the community to complete your visits.
Community Health Care Assistants perform phlebotomy, non-complex wound care and delegated insulin adminstration for patients with stable diabetes control.
The post is upto full time, worked as core shifts between 08:30-16:30. Weekends and Bank Holidays are worked as allocated.
