Senior Clinical Pharmacist

Job summary

We have an exciting opportunity for an enthusiastic and proactive pharmacist to join our supportive team delivering innovative pharmacy services to the community.

No two days will be the same; you will have patient contact; you will work as part of a multi-disciplinary team; you will problem solve and you will have the opportunity to shape the pharmacy service you deliver.

Prospective candidates should be efficient, enthusiastic and confident to work autonomously within a supportive team and their sphere of competence. They should have excellent communication skills as the job will involve liaising with social care, clinical and community teams as well as patients and their carers.

You will have a desire to improve patient outcomes; ensure our medicines governance is compliant with CQC standards, the will to learn and develop and the opportunity to influence the future of pharmacy services within the scope of the new NHS 10 year plan.

An independent non-medical prescribing qualification would be ideal. If however you are not currently a prescriber, you will be supported to undertake this.

You will need to be flexible in your approach to work and able to travel as the role involves hybrid working.

Experience in community services is not essential but a strong clinical background would be helpful.

This post is 30 hours a week.

Main duties of the job

You will be working as part of a multidisciplinary team supporting our four primary care home teams delivering patient centered care. You will also have the opportunity to provide clinical input into our two community hospital in-patient wards (Beccles Hospital and Carlton Court) as well as the specialist palliative care unit delivered by St Elizabeth's Hospice.

The Clinical Pharmacy Service must be safe, evidence based, innovative, costeffective and high quality.

You will be responsible for ensuring that there is compliance with medicines legislation; national and local guidance and to manage risks associated with medicines use. Audits, improvement projects and resulting action plans in line with the Pharmacy yearly plan will also be completed.

To ensure appropriate governance is in place regarding medicines, including leading on the response to any drug safety alerts or changes in clinical guidance, responding to incidents and escalate as appropriate.

To provide professional expertise, advice, support and training on medicines optimisation policies to health professionals.

To organise and participate in regular training and feedback sessions as appropriate to a variety of clinicians and support staff.

Mentor the Pharmacy Technicians and other pharmacy support staff and respond to their queries in a supportive and timely manner.

To deputise where appropriate (and agreed) for the Operational Lead or Head of Pharmacy.

To work closely with the pharmacy technicians to enable patients in our care to be empowered to manage their medicines.

To facilitate the medicines element of discharge in a safe and effective manner.

To participate in and undertake audits as agreed including collecting and recording data for KPIs.

To support the wider pharmacy and ECCH teams in delivering safe patient centred care through implementation of the Pharmacy and Medicines Optimisation strategy including providing professional advice; training and education; developing and reviewing relevant policies and SOPs.
