Job summary

Our nurse and therapist led Intermediate Care Unit offers a rewarding setting, focused on rehabilitation and reablement, with six Specialist Palliative Care inpatient beds. We use a team nursing and therapy approach, delivering person-centered care, with support workers working directly alongside qualified nurses or therapists in a collaborative environment.

This Band 3 role is ideal for experienced support staff. If you are less experienced, you can start as a Band 2 Rehabilitation Support Worker or Healthcare Assistant, with a clear development pathway to progress to Band 3 through training and supervised practice.

Please note: This is a 24/7 service and requires availability for nights, weekends, and bank holidays.

Main duties of the job

The post holder will be part of a dedicated team of support workers trained in a range of essential clinical and care skills. Working under the supervision of registered professionals, they will provide direct, hands-on care and contribute to delivering high-quality, person-centred care that supports both rehabilitation and end-of-life needs.

You will play an active role in helping patients regain independence, as well as providing comfort and support in the final stages of life. You will be expected to deliver delegated care and clinical interventions in line with your training and competencies.

Core clinical skills will include (but are not limited to):

Pressure ulcer prevention and management

Wound care

Catheter care and bladder scanning

Venepuncture

Recording clinical observations, including NEWS2 and blood glucose testing

Supporting physiotherapy and occupational therapy programmes within your scope of practice

This is a hands-on physical role ideal for individuals with a compassionate nature and a proactive approach to learning and development.

The Unit presently comprises of 20 intermediate care beds delivering rehabilitation and 6 Specialist Palliative Care (SPC) beds in a purpose designed setting. For our intermediate care patients our aims are to reduce acute hospital admission and facilitate early discharge by utilising the skills of the multidisciplinary team of nurses and therapists. For the SPC patients we aim to provide holistic symptom management and end of life care.

Support and deliver aspects of activities of daily living. To include management of dietary and fluidintake, toenail care, management of continence, assisting with toileting programmes, includinguse of sheaths, catheters, and continence pads.

Take, Record and Report (if outside of normal parameters) Blood Pressure, Respiration, Oxygen Saturation and Pulse rate as may be necessary using NEWS2. (National Early Warning System 2 Observation chart).

Undertake observations of skin assessments, continence, nutrition, and report back to senior colleagues.

Undertake urinalysis, including collecting urine sample as directed, reporting any concerns regarding urination.

Be able to participate in simple wound care, skin tear pathway, venepuncture, or collection of specimens.

Participate in Health Education/training for patients, families, and carers.

Engage in personal development; maintain competence, knowledge, and skills commensurate with role.

Be accountable for practice taking every reasonable opportunity to sustain and improve knowledge and competence.

Provide information as required to complete outcome measures on admission and discharge.

Maintain legible, accurate and contemporaneous patient records in accordance with ECCH Policy.

Report any changes in patients condition to a registered Health Care Professional.
