Job summary
An exciting opportunity has arisen for a Sister/ Charge Nurse/ District Nurse to join our Primary Care Home team delivering high quality nursing care, providing care at home to patients registered with GP surgeries in Waveney. The PCH team works closely with GPs and other members of the multidisciplinary team to provide an excellent standard of person centered nursing care to adult patients in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.
The successful candidate will be accountable for the effective management of a caseload, ensuring the delivery of high standards of evidence-based nursing care. Demonstrate excellent leadership, organisational and communication skills and the ability to influence and manage change.

Main duties of the job
The post offered is full time and sits in a multidisciplinary team providing community services 7 days per week 07:00-20:30 hours.
Applicants will be a registered nurse; with a minimum of 2 years community experience and a desire to develop.
The role contains variety every day with a mixture of face-to-face clinical practice, clinical triage, leadership and management. You will be responsible for the line management of several clinicians, have the opportunity to participate in HR processes including recruitment, appraisals and supporting junior members of the team.
The main duties of the role are: -
To provide clinical leadership, operational management and professional support within Primary Care home.
To work with members of the multidisciplinary team to provide expert clinical case management for patients living in the community who require planned and unplanned nursing care.
To work autonomously at a level appropriate to clinical competence and within scope of professional practice.
To carry out rapid assessments in patients own home with the aim of preventing admission to the acute hospital. These assessments will take a holistic ethos including physical, physiological and social aspects of need.
To be an autonomous practitioner that is responsible for own case load and to take a lead in the delivery of projects to improve service delivery, patient safety and quality of care.
Contribute to the continuous evaluation of the service to ensure it is fit for purpose
