An exciting opportunity has arisen for an Assistant Practitioner to join our Primary Care Home (PCH) to deliver high quality care to the population of South Waveney which includes Beccles, Bungay, Halesworth and Southwold. .

The PCH is a multidisciplinary team working closely with GP's and other community partners to provide an excellent standard of person centered care to adults in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.

Previous experience of nursing in the community would be beneficial but is not an essential requirement. We will support you to transfer your existing knowledge and clinical skills as an Assistant Practitioner working in a different care environment.

The PCH is commissioned to provide a 24/7 service. The successful applicant will be required to work 5 X 7.5 hours shifts between the hours of 08:30 and 16:30. The post includes weekend and bank holiday working on a rotational basis.

Main duties of the job

Within the PCH you will work in partnership with other members of the multidisciplinary team to provide planned and reactive community services. The post holder will provide clinical case management for a delegated caseload of patients living in the community who require nursing care.

The post holder is required to work autonomously at a level appropriate to clinical competence and within scope of professional practice.

The post holder will work under the supervision and working in partnership with a registered nurse, provide and deliver a high standard of nursing care contributing to the on-going assessment, provision and monitoring of care based on evidence and working in partnership with the multidisciplinary team.

  • Undertake and record MUST and Waterlow assessments and take appropriate action

  • Discharge patients from service
  • Arrange care packages in accordance with protocol
  • Complete Generic Assessment on System 1
  • Complete and allocate care plans and manual handling plans for patients.
