Permanent

Part time - 22.5 hours per week (Over 3 days per week)

Do you thrive on bringing people together to solve complex challenges? Are you passionate about making a difference for individuals who are struggling to engage with health and care services? If so, this could be the perfect opportunity for you.

We're looking for a Complex Case Pathway Coordinator to play a pivotal role in improving outcomes for adults experiencing self-neglect, particularly where non-engagement with health interventions is a key factor.

This post recognises the complexity of coordinating multi-agency responses and aims to strengthen collaboration between health partners across Bromley. You'll work closely with GPs, primary care teams, and other professionals to ensure that those at risk of self-neglect receive the right support, at the right time, from the right services.

Based within the Bromley Healthcare Safeguarding Service and line-managed within the Adult Safeguarding Team, you'll act as a bridge between agencies -- facilitating communication, arranging and minuting meetings, developing effective pathways, and helping to shape a more coordinated approach to care

If you're a proactive problem-solver with strong partnership-working skills and a passion for safeguarding, we'd love to hear from you.

Proposed interview date, week commencing 8th December 25

Main duties of the job

The Complex Case Pathway Coordinator will lead in the coordination of self-neglect cases, which are being managed through the Complex Case Pathway/ Framework.
