The post will primarily involve working with elderly patients and their families with a focus on goal-orientated discharge planning within their homes.
Interviews expected to be held on the 3rd and 4th June 2025.
We offer both full and part-time roles, providing 7-day service coverage across Bromley.
Main duties of the job
The individual in this position is responsible for safeguarding children, young people, and adults at risk. They will receive training at the appropriate level based on their role.
