£34,521 - £41,956 Inc HCAS Per annum pro rata
Fixed Term Contract - 12 Months
Part time - 22.5 hours per week (Over 3 days per week)
Acknowledging that single-agency coordination of multi-agency services around self-neglect cases is both challenging and resource intensive, the creation of a post for a Complex Case Pathway Coordinator seeks to support Health partners in Bromley in their response to self-neglect cases where the main cause of self-neglect relates to non-compliance with health interventions.
The Complex Case Coordinator role is funded by the South East London Integrated Care Board, but will sit in the Bromley Healthcare Safeguarding Service, with line management under the Adult Safeguarding Team.
A key element of this role, will be improving the response to complex self-neglect cases, through developing effective working relationships with Primary Care (GPs and Practice Staff), and other key professionals involved in the complex and diverse care of patients who are reluctant to engage with services.
Main duties of the job
The Complex Case Coordinator will lead in the coordination of self-neglect cases, which are being managed through the Complex Case Pathway/ Framework.
The role includes completing all administrative, information and organisational duties to ensure maximum outcomes from Complex Case Pathway meetings. The post-holder will be highly conscientious, organised, with excellent communication, planning and people skills in order to effectively manage the main duties and responsibilities set out below
