Specialty Doctor Community Medicine
This is a fixed term post in Community Medicine for 12 months with the potential to extend by mutualagreement. It incorporates General Internal Medicine within Hospital@Home and Rapid Response,Frailty, Rehabilitation, Long term and Palliative care.
The post is hosted by Bromley Healthcare (BHC) within the Urgent Community Response division(UCR). It will involve placement within the 'One Bromley' Hospital at Home service, the RapidResponse and Rehabilitation services. Additionally there is close working with the Bromley GP Allianceand Bromleag Care Practice (BCP) which provides GP services to Care Homes in Bromley. There isalso close collaboration with the St Christophers Team who provide Community Palliative Care.
There is scope to incorporate experience with One Bromley's Proactive pathway (providing Anticipatorycare planning and case management for complex patients, often living with frailty) and the Acute FrailtyAssessment Unit sited at the Princess Royal University Hospital (PRUH).
This post is suitable for clinicians from general practice, specialist trainees looking for an out ofprogramme experience (OOPE) and other suitably qualified doctors who are registered with theGeneral Medical Council.
Main duties of the job
The post holder will work across different areas of care where they will develop competencies in assessing and treating patients in the community.
Time spent in each area will be tailored according to patient flow, in order to optimise learning opportunities.

Bromley has the 2nd largest population of older adults in London constituting about 20% of the current population.The One Bromley Hospital @ Home service is an innovative healthcare initiative developed by the One Bromley Local Care Partnership which is a collaborative of all local providers commissioned to deliver NHS and local authority care. Hospital@Home currently provides Respiratory, Frailty, IV Antibiotic (IVAB) and Palliative services. This is due to expand further in the coming year. This acute level care for adult patients in their own home (or in a care home setting) means they can avoid hospital admission if this is not indicated.
The Rapid Response team consists of Advanced Nurse and Clinical Practitioners who provide 2 hour and same day response to adults who are unwell in their own home.
The service provides supportbetween 8am and 8pm, seven days a week to those who become unwell and who without this service may call an ambulance and make an unnecessary trip to hospital.
The Rehabilitation service is run by BHC and is responsible for 2 distinct pathways, Bed-based rehabilitation (sited at Foxbury Unit in Sidcup) and the Home pathway. Both pathways are multidisciplinary and include a dedicated Clinical Gerontologist.
The Acute Frailty Assessment Unit (6 trolleys and 6 Chairs) - a bespoke Acute Frailty Service aligned with local needs and demographics opened in 2021 at the PRUH. This service works with Bromley Hospital at Home, supporting patients to access necessary diagnostics and care while remaining in their usual place of residence. The aim is to be part of a fully integrated cross-boundary, cutting -edge centre for excellence in Gerontology and Frailty care across Bromley.
