Specialist Respiratory Nurse - Band 6

£40,701 - £48,054 Inc HCAS per annum pro rata

Full-time - 37.5 hours per week

This is a wonderful opportunity to join a well-respected, established multi-disciplinary respiratory team.

The successful candidate will work within the wider integrated respiratory team across the area to provide seamless care for patients with chronic lung disease. The team delivers expert care, optimising patients referred from GP's , which may include attending pulmonary rehabilitation which the team also provides. The successful candidate will also have the opportunity to become involved in the prescribing and monitoring of domiciliary oxygen.

Main duties of the job

To work as a specialist member of the respiratory team. This team provides expert knowledge and advice on the management of local patients with various respiratory conditions. The team works closely with other teams and secondary care to provide a fluid patient pathway.

The post holder will therefore maintain a high standard of patient-focused care reflecting an interdisciplinary approach to rehabilitation and self-management.

The postholder will manage a caseload of patients as an autonomous practitioner, including the maintenance of comprehensive records.

The management of highly complex patients in the community will require monitoring and linking closely with the Acute Trust.

To work as a specialist nurse within Bromley Healthcare Respiratory Service to manage clients referred to the Multi-Disciplinary Team-including complex patients

To assist in the provision of a high-quality and efficient service to all users and in auditing/benchmarking to evidence this.

MAIN DUTIES AND RESPONSIBILITES

  • To facilitate the pathway of care between primary and secondary care settings by developing strong links with secondary care to provide a seamless service for people with chronic lung disease.
  • To assess a patient with chronic lung disease holistically and develop an appropriate management strategy.
  • To focus on the prevention of hospital admissions by providing ongoing individual self- management plans to support optimal self- care in adherence to local and BTS/NICE COPD guidelines
  • To act as a resource and educator for patients, carers and their families to provide and advise on evidence based practices.
  • To provide effective specialist care, acting as a resource for the Respiratory Team and other health care professionals seeking advice on the management of patients with chronic lung disease.
  • To be able to recognise deterioration, or change in patients condition and provide appropriate care.
  • To support and participate, in the delivery of the Pulmonary Rehabilitation Programme.
  • To provide a high standard of care for patients.
  • To be responsible for ensuring the correct and safe use of the Community Respiratory Team Equipment used by patients.
  • To ensure that all data is collected and entered in an accurate and timely manner.
  • To participate in clinical audit and monitoring of the service to continuously evaluate the effectiveness of the team and to comply with Clinical Governance requirements.
  • To assist in the management, education support and review of junior members of the Community respiratory team.
