Bromley is an outer London borough with a diverse population where we are pioneering pathways to care for the local population by delivering NHS services using Neighbourhood teams integrated between Primary Care Networks and Bromley Healthcare. Exciting opportunities are available for cross-sector working in a variety of posts resourced via the 'Additional Roles Reimbursement Scheme' (ARRS).

We are developing a 'Neighbourhood' model as per the vision in the Fuller Stocktake report, with a focus on preventative and proactive care and helping patients see the right clinician at the right time as shown by our newly established Wellbeing Cafe.

We are seeking for some Patient Care Co-ordinators to join our amazing team.

The role of Patient Care Co-ordinator will work as part of an integrated neighbourhood team within Bromley PCN, being part of a team of clinicians and administrative staff focusing on preventative and proactive care. You will work closely with patients/service users and their relatives/carers, responding to queries and arranging appointments via a variety of IT systems. You will also be required to develop basic clinical skills such as blood pressure measurements, for which training will be provided.

  • 5 GCSE's at level 5-9 or A-C or equivalent.
  • A-Level/NVQ Level 3 OR equivalent

Experience

  • Previous experience of working in a health or social care setting, or another appropriate public sector role that has directly transferrable skills.

  • A basic knowledge of and understanding of (or a willingness to learn) health and social care terminology and familiarity with symptoms of medical conditions
  • Excellent communication skills (written and verbal)
  • Good interpersonal skills
  • Good organisational and planning skills
  • IT literacy including use of iPads, iphones & laptops.
