Community Nurse Associate - Rapid Response Team - Band 4

£29,460 - £31,909 Inc HCAS per annum pro rata

Full-Time Hours 37.5 per week

The Community Nursing Associate will be a member of a designated Community Team and is responsible for ensuring the effective holistic provision of high quality nursing care to people in their own home and a clinic environment who are registered with a Bromley General Practitioner.

Rapid Response is our 2 hours hospital avoidance service within the Urgent Community Response Directorate in Bromley Healthcare for avoiding unnecessary hospital attendances and admissions. The service provides diagnosis, treatment and where identified acute monitoring of patients for up to 5 days in their usual place of residence using both face to face and virtual care in the London Borough of Bromley. Operating 7 days per week 8am to 8pm.

Our service is looking for a proactive and enthusiastic associate nurse who is excited to be part of our team who are willing/interested to progress their career.Rapid Response in Bromley Healthcare (BHC) is an excellent and recognised urgent response service and promote growth and development as one of our BHC strategy for career ambitions. The role of the Nursing associate would be predominantly for urgent blocked catheter/bloods and simple assessment working within scope of practice. Driving is an essential component of the job.

Main duties of the job

1. To work within the Nursing Associate parameters of practice and deliver high quality, compassionate care to patients in their own homes/clinic environment

2. To work under the direction/delegation of a ANP/Senior Registered Nurse, in accordance with an individual care plan, with a focus on promoting health and independence

3. To carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of ANP, ACP, a registered nurse or other registered health care professional and at times support our sister UCR services such as Bromley hospital at home where required.

4. To proactively monitor, and prioritise patients visits within a delegated caseload, making autonomous decisions within agreed competencies about patient care and seeking support from colleagues as appropriate

5. Work in an effective and organised manner demonstrating excellent time management and organisational skills to effectively deliver person-centred care for an allocated group of individuals

6. To contribute to the development of staff within the service through training and supervision
