An exciting
opportunity has arisen for an experienced clinician (Paramedic, Nurse or Therapist) to join our expanding team.
The position is part of a developing service sat within Urgent Community Response
(UCR), covering the South Hams and West Devon locality and will help support
delivering 2-hour community response to vulnerable patients in-line with the
NHSE UCR directive.
The service provides 8am - 8pm cover 7 days a week and takes a
patient-focused multi-disciplinary approach in supporting patients to remain at
home and avoid unnecessary hospital admissions. The UCR service is expanding to support the creation of Virtual Wards, supporting more people to receive hospital care at home.
We would support applicants who are looking for a
development opportunity into an Acute Response Practitioner role. You would be
recruited into a band 6 role and on completion of recognised qualified (PACR
module, level 7) and competences achieve a band 7 Acute Response Practitioner
post.
Training and development
opportunities will be provided to successful applicants, in particular to
complete the advanced assessment skills module (on completion post becomes band
7) and develop their competencies to support both patients and the wider team.

Main duties of the job
The team work across South Hams and West Devon taking in a large rural geography. Pool cars, remote working as well as office bases across the patch are available to support dynamic working.
You need to have excellent communication skills and experience working with clinically vulnerable people. You will be adept at finding practical solutions and utilising a strengths-based approach alongside available local resources to support patients in the community. Candidates will need to demonstrate that they can gather and extract essential information to triage and respond appropriately, linking in with the wider MDT resources to ensure the patient receives the right care in a timely manner.
As an Urgent Community Response clinician you will be expected to:
- Support the UCR single point of access offering a clinician to clinician point of contact.
- Provide an excellent level of triage to quickly assimilate and hold risk.
- Be confident to assume responsibility for individuals & facilitate the handover of care from a referring clinician.
- Respond within 2hrs to an individual in crisis offering a comprehensive face to face assessment.
- Liaise & co-ordinate care with the wider MDT & stakeholders in a timely and effective manner.
- Play a key role in service development of both UCR and wider services in SH&WD.
- Work closely with stakeholders such as primary care, SWASFT & emergency departments to ensure UCR is promoted and developed in the most effective way possible.
