This project forms part of a wider plan to improve urgent and emergency healthcare delivery.
We are committed to revolutionising healthcare delivery for frail and elderly patients by launching a Community Frailty Virtual Ward. Our aim is to provide high-quality, patient-centred care in the comfort of their own homes, reducing hospital admissions and length of stay in hospital and enhancing overall well-being through enhanced care coordination.
Main duties of the job
We are pleased to advertise
this new post to support older people with frailty outside hospital beds. The Healthy Lives Partnership between
organisations in Plymouth has recently committed to specifically enhance the
care of frail people outside the acute hospital. We are setting up a community frailty team and
need your enthusiasm and skills with older people.
The right person for the
job will have and enthusiasm for excellence in the care of frail people and
support a broad multidisciplinary team including advanced clinical
practitioners in caring for a cohort of complex frail people in order to avoid
or shorten unnecessary days in hospital.
Your peer support will be
in our community physicians unified by the care of people, outside the acute
hospital, limited by frailty, neurological or severe illness. We are

patterns and interests.
