Job summary
Up to 25.5 hours per week
An opportunity has arisen for an experienced Occupational Therapist with diverse clinical skills to join our Community Therapy Teams, primarily based in the East of the city. We provide a patient-focused, multi-disciplinary approach to providing rehabilitation to patients in their own homes, supporting discharge from hospital, maximising independence and preventing admission . This also incorporates the Livewell Referral and Support Service which triages and prioritises all community therapy referrals for Plymouth.

Main duties of the job
We are divided into four locality teams and have a strong emphasis on evidence-based practice, in-service training, individual performance review and personal development planning. We provide excellent support through regular line management meetings, post-graduate education, clinical supervision and strong academic links with the University of Plymouth, which also provides an undergraduate OT course
The successful candidate will be an excellent communicator, competent in a wide range of clinical areas including neurology, orthopaedics, elderly rehabilitation, long term condition management, DFGs and complex manual handling. They will demonstrate flexible problem-solving skills, working alone or with colleagues from the multi-disciplinary team
The team benefits from close working relationships with GP practices, Universities Hospital Plymouth (Derriford) and other local hospitals focusing on acute care and rehabilitation. We have close links with urgent care services, preventing hospital admission and supporting discharge into the community. We have parallel working with condition specific services, and have a service focus upon long term condition management
