exciting Acute Virtual Ward project. This project forms part of a wider plan to
improve urgent and emergency healthcare delivery, across Plymouth, the South
Hams, and West Devon.

frail and elderly patients by launching a Community Frailty Virtual Ward. Our
aim is to provide high-quality, patient-centred care in the comfort of their
own homes, reducing hospital admissions and length of stay in hospital and
enhancing overall well-being through enhanced care coordination.

Main duties of the job
The Role
As an Advanced Clinical Pharmacist, you will be a
cornerstone of healthcare delivery and will play a critical role in the
multidisciplinary team, covering the Acute Virtual ward that is in operation in
our locality.
Here is what you will do
- Leadership within the Virtual ward regarding
medicines and medicines governance.
- Medicine reconciliation.
- Structured medication reviews including complex
polypharmacy and deprescribing.
- Prescribing and accurate record keeping
regarding medications.
- Answering medication queries raised by the
multidisciplinary team and providing clinical support to the Acute Virtual Ward
Team.
- Discharge facilitation.
- Work within integrated care across several
multi-disciplinary teams including, Mental Health, District Nursing Discharge teams,
Acute GPs, and Social Workers.
