Job summary
This post will involve competencies which will need to be
completed within 2 years dependent on current experience and will be assessed
through various methods including feedback, self-directed assessment, direct
observation and throughout line management. If not achieved within the
specified timeframe, the postholder will not be able to progress onto the band
6.
The postholder is
responsible for the assessment of care needs and the development,
implementation and evaluation of programmes of care.
He/she will be
designated to take charge regularly of the ward.
The postholder is
required to supervise staff and be able to teach qualified and unqualified
staff, including basic and/or post basic students.
The
Clinical Team Leader will at all times work within the spirit of the NMC code
of Professional Conduct for trained nurses and local policies.
All Livewell Southwest staff are expected to able and willing to work across a 7 day service.

Main duties of the job
To be a member of a multi-disciplinary
team, contributing in all its forums, attending meetings and case conferences
for patients.
To link closely with Key Workers in the delivery of care under the Personalised Care Framework.
To promote a collaborative approach to
patient care within the team providing support to junior staff.
To assist the Ward Manager in the
introduction Quality Improvement Projects and Culture of Care Project and in the introduction of an effective care delivery system devised to meet the needs of patients which is evidenced based wherever possible.
To act as clinical role model,
demonstrating high standards of nursing practice.
To assist the Ward Manager and Deputy
Ward Manager in the introduction of patients involvement in the day to day
issues on the ward.
To ensure close links with other
Glenbourne wards/departments to identify/resolve clinical issues.
To be responsible implementing drug
administration procedures according to laid down policies.
Actively participate in clinical
governance and quality improvement initiatives, e.g. clinical audit, essence of
care, or clinical policy development.
Provide, receive and analysis complex
information from a range of sources.
Analysis and make decisions in relation
