Are you an experienced nurse passionate about frailty and older peoples care? We are excited to offer the opportunity for two Band 6 Registered Nurses to join our innovative and expanding Community Frailty Virtual Ward (CFVW) team.

The CFVW is community-based, operating across six localities in Plymouth, South Hams, and West Devon. We provide face to face proactive and reactive care to patients living with frailty and at risk of deterioration or hospital admission. We aim to support patients to remain well and independent at home, avoiding unnecessary hospital stays through Comprehensive Geriatric Assessment (CGA)and collaborative care planning improving outcomes and quality of life for our most vulnerable population.

Our Multidisciplinary Team includes:

  • GPs and Advanced Clinical Practitioners (ACPs)
  • Clinical Pharmacists
  • Frailty Practitioners
  • Physiotherapists and Occupational Therapists
  • Health Care Support Workers

We have identified the need to strengthen our team with skilled Registered Nurses who bring expertise in frailty or older peoples care, advanced care planning, background or interest in the management of heart failure and are committed to delivering high-quality, patient-centred community nursing.

All Livewell staff are expected to be able and willing to work across a 7-day service.

Main duties of the job

To manage and prioritise a defined caseload and delegate where appropriate to the skill mix of the team.

As part of your role there may be a requirement / opportunity to undertake Treatment Escalation Plan (TEP) discussion, to achieve this you will be required to be competent in advanced communication, care planning and attended appropriate training to support competence.

To act as a role model for other staff and students demonstrating high standards of specialist practice and professional conduct.

Be responsible for providing and maintaining a learning environment and maximise opportunities for educational and development in the clinical area to enhance individual development and performance in the delivery of high standards. Acting as expert resource and assist in teaching knowledge and clinical skills for other health care professionals.

Take part in the process of clinical supervision and support clinical supervision and reflective practice for team members.

To develop strategies for continued professional development as Non-Medical Prescriber if relevant for role

Support recruitment and selection of new staff, which may include being part of an interview panel.

Act as mentor for new staff and students, including leading on a structured induction and training program for new members of staff.

This role may not be eligible for sponsorship under the Skilled Worker route.

Be responsible for the provision of planned and unplanned care to patients and their carers in a defined geographical area working as part of the 6 Community Virtual Wards spanning Plymouth, South Hams and West Devon.

Be able to respond to urgent referrals for patients, carers and the multi-disciplinary team

Be responsible for assessing, diagnosing, planning, organising & reviewing complex packages of care including case conferences

The candidate must have the knowledge and confidence to have delicate conversations around advance care planning in anticipation of end of life care including treatment escalation and have the skills required to make clinical decisions around resuscitation status based on the persons clinical presentation.

Ensure privacy, dignity and human rights are respected for all patients and carers.

Undertake comprehensive risk assessments of all situations associated with the care of patients in order to ensure nurses and the carers safety

Assessments might include the provision of equipment ensuring it is used safely

Be responsible for overseeing, monitoring of equipment and ensuring maintenance &repair.

To report and records all incidents and near misses relating to health, safety, security, fire, physical violence, aggression and verbal abuse.

Maintain accurate records, which are confidential, up to date , legible and that all care given has to be documented. These records may be paper or IT based system and must be maintained as specified in the Trust Policies, NMC guidelines and Government directives. The post holder will be expected to oversee the standard of patients records by audit or peer review and have a regular open discussion with team members.

The post holder is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations or Chief executive office

Be responsible for organising own time management on a daily basis in line with caseload demands balancing patient needs with the non clinical aspect of the role

Work within challenging and unpredictable environments e.g. patients homes, which may have constraints that require adaptability

Undertake health promotion whenever possible to all age groups and also contribute to planned health promotion activities.
