37.5 hours per week.
An exciting opportunity has arisen for an experienced, innovative, forward thinking Registered Nurse to become part of the team in the Community Neuro Rehabilitation Team based at Mount Gould Hospital, Plymouth.
We are looking for a registered nurse who is motivated and flexible in their approach and able to demonstrate excellent clinical assessment and rehabilitation of neurological patients and management/leadership skills.
The successful applicant will be joining a friendly team working alongside experienced therapists and supported by the Neuro Rehab Matron and multidisciplinary team leads; ensuring the provision of high quality patient centred care and rehabilitation.
The post holder will lead on specific projects and be involved in the development of new pathways and ways of working. You will receive regular practice supervision and have the opportunity to undertake quality improvement tasks.

Main duties of the job
The post holder will be working in collaboration with otherCNRT leads and MDT managers in planning, coordinating, delivering, developing andevaluating the service provided to people on a day to day basis.
Support timely discharge fromhospital whilst assessing and supporting ongoing rehabilitation goals to maximise a patientspotential within their home/community setting, while supporting families and carers during thisepisode of care.
Ensuring safe staffing levels, acting as an expert resource for the nursing team and supportingthem in the delivery of evidence-based care.
Leading by example and empowering the multidisciplinary team in their personal and professionaldevelopment to undertake a range of clinical skills to modernise and improve patient care, includingupdates of essential training.
To maintain high professional standards and continuously promote/develop quality improvementappropriate to the needs of the service.
Support self and service development through active participation in continuing professional development.
Make recommendations and lead on any changes in practice within the team and contribute to the development of service protocols, policies and procedures.
Supervise, educate, and assess the performance of junior staff.
All staff are expected to be able and willing to work across a 7 day service.
