to worth within a new model of care across Plymouth, through the newly established
Health Inclusion Pathway-Plymouth Team
In line with current best
practice this service has been developed using the Pathway approach,
providing end to end support for individuals who are experiencing homelessness
and health and social complexities, including substance use issues, mental
health issues and a history of poor engagement with services.
The Health Inclusion
Pathway-Plymouth team is a multidisciplinary team, with doctors, occupational
therapists, physical and mental health nurses, plus support staff.
The aim of the Inclusion Health
service is:
To create a comprehensive
service, for adults with complex needs related to homelessness, health, contact
with the criminal justice system and substance misuse issues.
The Service aims are:
Admission Avoidance: preventing inappropriate admissions, through sustained
proactive work in the community with services, including homeless hostels and
drop-ins.
Inpatient
Support: supporting those people from
our cohort to remain in hospital, until completion of treatment.
Safe Discharge from
Hospital: aiming for discharge from
hospital, not just when treatment is completed, but when a person can safely
return to the community.
All LW staff are expected to be able and willing to work across a 7 day service.

post holder is a key player in influencing system change within the
organisation including taking a lead on teaching/promoting inclusion health within

independence and to ensure they establish the appropriate level of support to
prevent admission into hospital and maintain people in the community.

expert skills and knowledge to the service and be a role model for the team,
striving to deliver inclusive provision.
Conduct
health and care needs analysis of the caseload within the service, identifying
unmet need, developing service provision and effective discharge plans.
The role will involve working within a
Multi-disciplinary Integrated Health care team which will enable close working
partnerships with healthcare and professionals leading to positive outcomes for
people supported in the community.
To work alongside the Operational Lead on the delivery of specific service
targets as identified through audit, by the commissioners, and the Pathway team.
This role may not be eligible for sponsorship under the Skilled Worker route.

- Social Work Degree.
- Evidence of Continuous Professional Development by producing documentation to meet the Standards of Proficiency for Social Worker registration
