Job summary
37.5 hours per week. The post holder will work in the Health
Inclusion Pathway-Plymouth multidisciplinary team as an autonomous
practitioner. This is an exciting opportunity to provide a new model
of care across Plymouth, through the establishment of the Inclusion Health
Service.
In line with current best practice this service is
being developed using the Pathway approach, providing end to end support
for individuals who are homeless. It involves not only medical staff, but a
range of multidisciplinary professionals with expertise in social care, housing
law and benefits issues, ensuring that a persons full range of needs are
supported.
The team works across areas/units/wards within Plymouth hospitals and a wide range of community settings. Our aims are to avoid inappropriate and frequent hospital admissions, support our client group during admission to hospital and to facilitate safe discharge, from hospital to the community.
Main duties of the job
To develop and oversee
the provision of an evidence-based occupational therapy service for adults with
complex needs related to homelessness, health, contact with the criminal
justice system and substance misuse issues.
To provide highly specialist occupational
therapy assessment, formulation, and interventions to adults with high levels
of complexity and vulnerabilities.
To provide specialist advice, supervision,
case discussion & formulation to staff supporting adults who have a diagnosis
of a severe and enduring mental health issue.
To provide specialist occupational therapy
input to planning, development and evaluation of clinical services within the
mental health services, holding responsibility for defined projects.
To support robust risk assessment and
management, offering specialist plans and intervention which support
individuals within the community setting.
To
develop and sustain partnership working with community agencies and
facilities.
To promote inclusion, awareness of
occupational performance issues and onward support.
To make autonomous clinical decisions about
own professional practice.
