Job summary
An exciting opportunity has arisen for a highly motivated and dynamic Band 6 Occupational Therapist to join the Discharge to Assess (DTA) Therapy Team based at Mount Gould hospital, Plymouth. The DTA service is a large multi-disciplinary team that consists of occupational therapists, physiotherapists, nurses, social workers, community care workers and support workers all working together in the community city wide.
The DTA team support safe and timely hospital discharge of clients who are medically fit by using the Home first approach. Assessment is completed within 2 hours of discharge and care provision can be tailored with a reablement focus to support individuals to regain strength and skills to live as independently as possible. DTA Therapists work in partnership with individuals and families identifying needs and goals, using a person-centred and strength-based approach to care planning. The Therapy service also support work with bedded pathways and assist with multi professional assessment for eligible needs under relevant legislation.
The Occupational Therapy role is essential within the team and incorporates a wide variety of opportunities to support development of knowledge and skills required for post.

Main duties of the job
The DTA model of work aims to support improved outcomes for people leaving hospital by:
Reducing the time people spend in hospital when acute care no longer required.
Assessing people in an appropriate environment giving a more accurate indication of their strengths and needs.
Providing multidisciplinary reablement and rehabilitation plans, and if necessary short-term care and support, to help people improve independence, preventing or reducing need for longer term care.
The model enables the urgent care system to prioritise acute hospital care for those people who need it.
The Therapy team work closely with:
Individuals and families.
Wider MDT and therapy colleagues within acute and community care.
Other local services as part of the Urgent Care System supporting arrangements for discharge to one of the two pathways within the DTA service; Home First and Bedded Pathway.
Voluntary sector / local agencies.
All Livewell Southwest staff are expected to able and willing to work across a 7 day service.
