Job summary
This is an exciting opportunity for an experienced and self-motivated nurse to be part of a developing Urgent Care Nursing service working across Plymouth, South Hams and West Devon. This is a permanent role of up to 37.5 hours per week, within a 24 hour service.
The key part of our service is providing high quality nursing care, to meet the need of the individual in the home, supporting the individual to remain at home, safe and warm, to stay independent maximising wellbeing and improving health outcome through to End of Life care.
You will also have the opportunity to play a role in the future development of the service and will work alongside other colleagues in delivering person centred care.
You will have the opportunity to work with a wide range of the multi-disciplinary team, including; Adult Social Care, Community Therapy, CCRT and the District nursing service. Working together in a culture based on; compassion, empathy with respect and dignity.

Main duties of the job
The post holder will deliver a range of clinical skills and expertise providing care in the community. Primarily the role involves working within the Day IV service, visiting patients within their own home to deliver various intravenous therapy between the hours of 0800 and 2030hrs.
The post holder is responsible for the delivery of high quality evidence based care for acutely ill patients within their homes, with the aim of preventing hospital admission or facilitating rapid discharge.
The post holder will liaise with the referring clinician when accepting patients for treatment and be supported by senior clinicians and clinical manager.
Responsible for effectively assessing, planning, implementing, monitoring and evaluating patient care this will include admitting and discharging patients form the caseload.
To prevent and reduce hospital admissions and minimise hospital lengths of stay by providing intensive and advanced nursing input, and involving appropriate members of the multi-disciplinary team.
Responsible for coordinating a shift in the absence of a senior clinician.
