Livewell
Southwest (LWSW) is seeking a Senior Rehabilitation
Health Care Assistant in the William & Patricia Venton Centre for up to 37.5 hours per week. This is a new 23 bedded in-patient unit for adults who require intermediate care to avoid
hospital admission or are clinically fit for discharge from hospital but require
social care or support with reablement goals before returning home.
You will
need to be dynamic&responsive to the needs of the patients across the WPVC
site, this will involve contact with relatives, carers &other agencies. You will support people to engage with a personalised
rehabilitation program &actively encourage engagement in everyday life
skills, promoting independence &rehabilitation in a safe manner and in line
with assessed risk. Interventions will be carried out as part of the care plans
and information recorded as directed by LWSW's policies &procedures on electronic patients notes under the supervision of the
registered health care professional.
There
are a range of opportunities to learn new clinical skills &opportunities to
progress. You will undertake an induction as well as specific training relevant
to the areas you may be allocated to, this includes continence, tissue
viability &pressure area care.
All Livewell Southwest staff are expected to able and willing to work across a 7 day service.

Main duties of the job
The role of the Senior Rehabilitation
Health Care Assistant is to assist the
trained member of staff in delivering physical, social & psychological care
to the patient/client with physical/mental health problems/illness,
admitted to the WPVC.
The post holder will be dynamic &
responsive to the needs of the patients across the WPVC site as directed by
the senior manager.
This will involve contact with
relatives, carers &other agencies. They
will carry out interventions as part of the care plans &record information
as directed by LWSW's policies &procedures on electronic
patients notes under the supervision of the registered health care
professional.
The post holder:
Will have effective
communication skills &dynamic risk management skills
Support
service users to engage with a personalised rehabilitative program.
To actively encourage &promote engagement in everyday life skills, promoting independence &rehabilitation
in a safe manner &in line with assessed risk.
Provide support as
described in care plans &risk management plans.
You will be based within
the WPVC for managerial support &line management.
You are required to be flexible in the shift patterns as service
requirements can vary depending on patient need & could vary in length
& include nights.

13.To assist colleagues, under direction, with tasks which fulfill the patients / clients physical and psychological need, as identified in the Care Plan / Assessment.
14.Escort patients to other wards / departments or other areas designated by trained staff, which may include the community.
15.Contribute to the assessment, planning, implementing and evaluating care to patients. This to include participation in case review and assessment for admission / discharge process.
16.Make appropriate records in the multi-disciplinary patient file / electronic records, which will then be counter signed by a registered nurse.
17.To assist the patients personal care and record patients observations in accordance with the Care Plan.
18.Record and manage patients property.
19.Undertake specific responsibilities designated by the Service Manager or staff member in charge.
