Job summary
22.5 hours per week. An opportunity has arisen for an experienced and committed mental health, learning disability or social care professional to be part of the IPP team to work as a practitioner alongside the mental health in-patient teams to ensure that people who have needs related to mental health, learning disabilities and autistic spectrum disorder and who require a discharge plan, are not delayed in hospital, and that S117 care packages are robustly reviewed.
The post will have a base at Glenbourne, however will work from home and across the city. The successful candidates will have experience in ensuring a high standard of assessment of needs and a proven track record in developing outcome based objectives with service users. They must be motivated, able to liaise and negotiate with a wide range of stakeholders and work collaboratively with Multidisciplinary teams, Care Coordinators and Lead Professionals in Mental Health and Learning Disability teams city wide. They should have a good working knowledge of the mental Health Act 1983, the Care Act 2014 and safeguarding adults, Children and Young people. We are looking for someone with a desire to work alongside service users and carers to promote recovery, encourage independence and foster hope. Main duties of the job
This post will be part of a team, managed by the IPP/117 Manager, who will deliver the key objectives of Livewell Southwest. That is, that wherever possible Plymouth residents who have needs related to mental health, learning disabilities and autistic spectrum disorder, for whom commissioning responsibilities sit with PCC or Devon ICB and who are entitled to section117 aftercare or have a funded individual person placement, live in their own homes and are not in Out of Area (OOA) hospitals and placements, a significant distance from their local area.
Work as a clinical practitioner alongside the mental health in-patient teams to ensure that people who have needs related to mental health, learning disabilities and autistic spectrum disorder and who require a discharge plan , receive the right package of care, at the right time in their own home or an appropriate setting. The package of care should not be overly restricted, should take into account the person or their representatives choice and wishes, be of a good quality and cost effect.
That S117 Care packages are robustly reviewed within an agreed timeframe, to an agreed framework, to make sure they are good quality, are meeting peoples needs and are value for money.
To ensure that service users who are entitled to 117 after-care, receive the support and treatment that they need for as long as they need it.
