Job summary

The complex needs team are looking for a community support worker to join the multidisciplinary team, based at Harbour substance misuse services. Working with people who experience mental health difficulties such as psychosis, and mood disorders, alongside substance misuse issues.

This is an opportunity to work across services such as Harbour, Homeless provisions, community settings and clients homes. Working closely with the community mental health framework to engage clients in person centred care.

This role will be working in a multidisciplinary team of mental health nurses, social workers, assistant practitioners, Nursing associates and doctors. Regular team supervision and opportunities for in-house and national training opportunities are available for all staff.

Main duties of the job

The role will require the post holder to work across the complex lives system, developing in consultation with service users flexible and realistic support packages and person-centred plans within agreed guidelines and service models.

This post is seen as crucial in working alongside Registered staff to develop and implement person centred care plans in order that the person using the service can optimise their level of functioning despite any barrier their complex problems may present.

Care and recovery support is delivered to clients in their homes and within a variety of accommodation settings and will be monitored with the aim to optimise service users recovery needs.
