Job summary

27 hours per week. Come and join our Community Nursing Service that covers the beautiful South Hams including Yealmpton, Ivybridge, South Brent areas. The teams provide nursing care to patients within their own homes with emphasis on preventing hospital admissions and delivering care within local communities. The teams promote health and well being for living well. The role is rewarding applying your skills to make a difference.

Main duties of the job

The role is supporting the District Nursing Service in delivering care to those who are recovering from surgery and ill health and those nearing end of life. The District Nursing Services visits patients in their own homes and provides nursing care alongside our MDT colleagues. The Health and Social Care team provide an integrated service with help from social prescribers and voluntary services.

The nursing service provides a wide range of clinical care and health promotion.

Experience

Essential

  • Experience of working within a nursing team
  • Experience of dealing with vulnerable patients

Desirable

  • Previous Community Nursing experience
  • Wound care and lower limb management experience
  • Experience of medication administration

Qualifications

  • Qualified to NVQ level 3 in Health and Social Care or Similar Qualification

Desirable

  • Qualified in Phlebotomy
  • Wound Care experience
  • Previous Community Experience an advantage
