Job summary

26.25 hours per week. An exciting opportunity has arisen for an experienced Paramedic/OT/ Physio/ Nurse to join our expanding team and become an Acute Response Practitioner. The position is part of our developing Urgent Community Response (UCR) service & Virtual Ward covering the South Hams and West Devon locality. The service provides 8am-8pm cover 7 days a week and takes a patient-focused multi-disciplinary approach to support individuals to remain at home & avoid unnecessary hospital admissions.

The successful candidate will need to have excellent communication skills. We are looking for someone with experience in working with clinically vulnerable people, finding practical solutions to crises and the ability to utilise all available local resources to support patients in the community. Candidates will need to demonstrate that they can gather and extract essential information to triage and respond appropriately. You will be expected to build relationships with the wider MDT to ensure that your patient receives the right care, at the right time in the right place.

If the successful candidate does not hold the correct advanced clinical skills module/ qualification they will be supported to complete an appropriate course. They will be employed at a band 6 level until they have passed the course.

Main duties of the job

As a UCR clinician you will be expected to:

- Support the UCR single point of access offering a clinician to clinician point of contact.

- Provide an excellent level of triage to quickly assimilate and hold risk.

- Be confident to assume responsibility for individuals & facilitate the handover of care from a referring clinician.

- Respond within 2hrs to an individual in crisis offering a comprehensive face to face assessment.

- Respond to non-injurious fallers

- Liaise & co-ordinate care with the wider MDT & stakeholders in a timely and effective manner.

- Play a key role in service development of both UCR and wider services in SH&WD.

- Work closely with stakeholders such as primary care, SWASFT & emergency departments to ensure UCR is promoted and developed in the most effective way possible.
