Leader to join the Urgent and Intermediate Care Service in the role of Integrated
Operational LeadThis new
post has arisen out of the Shaping Our Futures Programme, which aims to enhance
integrated working across health and social care teams within Adult Frailty and
Specialist Services.
The Urgent and Intermediate Care Service is key to ensuring
that people are able to remain living at home with as much independence as
possible for as long as possible, to preventing unnecessary admissions to
hospital or long term care, and to support timely discharge from hospital.
This post will work alongside the Deputy Heads of Service in
UIC, and our Band 7 Nursing, ASC and Therapy Leads to deliver the objectives
and support good outcomes for people using the service. The post holder will
have knowledge and experience of working within Urgent and Intermediate Care,
will demonstrate excellent leadership skills and the ability to work with
services, teams, staff and service users to develop and improve systems and
structures.
Being a new post, it is essential that interested candidates
are able to work flexibly and creatively in a changing environment, are able to
use their initiative and work autonomously, as well as working as part of a
team, and are willing to undertake a range of tasks and roles as the functions
of the post and service develops.
Main duties of the job
- To provide non-clinical leadership for staff within both
services. Ensure that the staff teams are capable of meeting the needs and
objectives of the service by ensuring the functions are fit for purpose
and new ways of working and relevant organisational policies are implemented.
- To be the budget holder for the integrated team budgets,
responsible for proactively managing the budgets by
analysing and monitoring expenditure to achieve balance, identifying any
deviations from the norm and planning to address these.
- The post holder is accountable for the direct
delivery of the integrated teams ensuring they are person centred, responsive,
adaptable and are provided according to patient need and available resource.
- To ensure there are robust
multidisciplinary/unidisciplinary team processes in place within the services
that include a timely response to referrals, initial assessments and formulation
of care plans .
- To ensure that effective methods of measuring service performance
and quality standards against agreed quality indicators, including implementing waiting time targets, and are regularly reviewed.
