An exciting opportunity has arisen at Livewell Southwest for any band 5 Nurseswho currently do not hold a driving licence -we can support you to learn to drive by providing lessons during working time.

LWSW appreciates that work life balance is vitally important for our employees wellbeing & therefore we have hours available from 22.5 - 37.5 hours per week, working across 7 days.Our core hours are 0800 20:00 & are open to discuss any working pattern within these hours, to enable you to get the balance of home & work life that suits you.

The role requires positivity, excellent communication, leadership, & caseload management abilities. You will need to demonstrate integrity, initiative, sound clinical knowledge with evidenced based practice & the ability to support the team, approaching your work in a coordinated way.

We work integrated with primary care, adult social care, therapists, urgent & intermediate care services alongside our specialist services informing our ageing well MDTs which facilitates shaping our future to deliver great quality care for housebound individuals.

If you have an interest in community nursing, preventing hospital admissions & caring for people with complex long-term disease, through to the last stages of their life, this role is for you.

Main duties of the job

As part of this post you will also get your driving lessons paid for during working hours which can be flexible with the expectation that you have to remain in post for 1 year.

You will be joining the District Nursing Clinical Support Team (DNCST) where you will be driven to your designated areas for drop off & pick up by either a community healthcare professional or a designated driver. Whilst learning to drive, we will transport you to &from your patients within a DNCST car driven by a competent driver until you have completed your driving course.

You will be required to undertake a robust induction, allocated a mentor to support your learning to develop in your role. We offer training in line with the required competencies for you to build on your knowledge, experience &skills. You will be expected to undertake continued learning with accessibility to accredited courses.

You will be required to be an autonomous practitioner & a role model. Be responsible for assessing, triaging, planning, implementation &evaluation of care, support the Community Sister/Charge Nurse in their absence.

To be a Community Registered Nurse your role is vital in terms of coordinating peoples care through holistic evidence-based best practice in accordance with National & Organisational approved polices/procedures.
