Key functions of the role include the operational leadership for teams and provisions which support admission avoidance and discharge pathways in South Hams and West Devon.

This is also an opportunity to take leadership of developing the Urgent Community Response across Western Devon and Plymouth.

You will lead on development of integrated working with primary care, community and acute partners.

You will also be asked to deputise for the Assistant Director (Western Devon) when appropriate and represent both Devon County Council and Livewell Southwest.

You work closely with other urgent care and community services managers across Plymouth and Devon.

This role maybe asked to support other development work as required.

Main duties of the job

Leadership, Policy and Service Development

  • Act as strategic lead for the transformation of services for intermediate care, long-term condition management and frailty; challenge current structure and identify organisational and professional barriers which limit or inhibit the service. Lead and foster a culture of openness and recognition of duty of candour in which each member of staff is valued, supported and developed, inspiring a shared purpose to the delivery of high quality and best value health and social care effective care.
  • The postholder will have a key role in crossing organisational and professional boundaries, linking together multi-agency services for appropriate discharge care pathways in response to national initiatives and advising on appropriate clinical and professional strategy relating to therapy practice and the older person. In particular, the post holder will have a key role in reducing delays transfer to care and facilitating the identification of discharge pathways for stranded patients.
  • To assist in leading a local management team with accountability for delivering key targets, including quality outcome indicators, performance measures and financial balance on behalf of Livewell Southwest and Devon County Council..
  • To be part of the operational management team, overseeing the delivery and development of Livewell Southwest and Devon County Council. Lead multi-professional and multi-agency working towards the successful development of complex packages of care for discharge, which promote early discharge/reduced length of stay and /or prevent unnecessary hospital readmission - working in partnership with colleagues in the community and primary care. Act as liaison and lead for Community teams, Care Homes and Community Hospitals to promote improved care pathways for patients.
  • To be a member of senior management groups as required (e.g. workforce, risk management etc.).
  • To build relationships at a local level which deliver integrated health and social care across services.
  • To assist in the completion of projects and prepare and deliver reports and presentations as appropriate.

  • Work with strategic and operational/professional leads, service mangers users/carers to ensure that good practice, policies, protocols and service development plans are implemented at a local level.
  • Shape and develop the nature and range of services and pathways provided in community and hospital settings.
  • To promote the principle that the people who use our services, and their carers, should be at the centre of all we say and do in all contacts and interactions with staff, people who use our services, carers, partner organisations and members of the public.

Financial and Physical Resources

  • To work with the Locality Manager and Head of Servicer and assist in taking responsibility for budgets across a complex range of services.
  • To have full devolved responsibility for relevant budgets in line with service needs and objectives.
  • To be responsible for the monitoring of devolved budgets in defined locality area.
  • To assist and support and contribute key factors in the delivery of financial balance and to include financial recovery as required in line with sustainability and QIPP targets.
  • To ensure that risk management polices and procedures are implemented, in the defined area, and that risks are managed/mitigated locally or escalated to an appropriate level in the organisation.

Support, Management of Development of Staff

  • To assist the Locality Manager and Head of Service in the support, and development of staff.
  • Develop the multi-professional discharge ward team to provide expert advice on the specialist assessment and planning of discharge needs for people with multiple pathologies, complex needs, polypharmacy, infection risks, mental capacity needs, reduced functional abilities and safeguarding needs; developing an advanced level of professional autonomy and accountability in the planning of complex discharge
  • To ensure that all teams involved in Community Services Urgent Care are consistent in their approach to supporting and developing staff.
  • The post holder will provide leadership and management for all staff within Community Services for the delivery of integrated care provision to ensure that all potential and acute service users receive a consistent and comprehensive service outcome.
