Job summary

30 hours per week. We are looking for a highly skilled, experienced, and enthusiastic Cardiac Rehabilitation Nurse to work in the Livewell Southwest Community Cardiac Team.

This 1-year post has been created to support and improve Cardiac Rehabilitation by offering Pre-habilitation to patients in University Hospitals Plymouth. Improving Cardiac Rehabilitation provision is currently high of the agenda of The Integrated Care Pathway review and this post is part of a pilot scheme that will provide evidence to highlight the requirement of Cardiac Prehabilitation in the future. There is also a requirement to support the delivery of Phase 1 Cardiac Rehabilitation on the in-patient wards.

This role will evolve with the pilot and offer potential opportunities in the future.

Managed and supported by The Community Cardiac Team in Livewell Southwest, you will be an educational resource, support and advocate for cardiology and cardiothoracic patients who currently not offered any Pre-habilitation at all. This successful candidate must be a confident communicator, skilled motivator, and work well independently and within the multidisciplinary team.

Main duties of the job

To undertake a leading role in the planning, coordinating, delivering and evaluating of high quality Cardiac Nursing care for patients with Cardiac Pre-habilitation needs. This will be focused primarily on planned CABG patients.

To provide Phase 1 Cardiac Rehabilitation to patients on the acute ward.

To act as a resource of specialist expertise for patients and the wider MDT across Derriford and Livewell Southwest.

To provide an effective interface between the patient and Primary and Secondary care and offer a seamless, consistent, and cohesive service.

This role may evolve into other aspects of cardiac rehabilitation where the need arises.

  • Evaluate the impact of these training programmes, for patients and carers, to ensure that they provide the necessary knowledge and skills to gain independence, safely manage changing circumstances and plan for unavoidable progression of conditions
  • Be responsible for providing and maintaining a learning environment and maximise opportunities for education and development in the clinical area to enhance individual development and performance in the delivery of high standards of care

Risk Management
