Job summary
We are looking for registered nurses to work for us as part of our Integrated Care Teams and will participate in the assessment, implementation and evaluation of care to patients in their own homes and will be committed to the development of the Primary Health Care Team.
We have vacancies available in the following locations
- Maldon ICT (this team covers 4 areas- Maldon, Witham, South Woodham Ferrers and Burnham on Crouch. You will be expected to cover all areas if necessary, your preference of base will be noted however not guaranteed.
Main duties of the job
The post holder will work across the planned and unplanned pathways delivering care to housebound patients. They will act as a professional and clinical role model demonstrating and upholding the values of the organisation and profession as well as behaviours to facilitate a culture of trust, openness, integrity, kindness, and mutual support. They will act as an ambassador for Provide and demonstrate excellent customer care.

Within the planned pathway the post holder will support the effective management a defined caseload of housebound patients in the provision of quality nursing care, applying evidence-based practice.Within the unplanned pathway the post holder will respond according to clinical priority for patients triaged as requiring care outside of the planned pathway.
This includes attending the patient, where possible, within a four-hour period. It is expected that staff working on the unplanned pathway will liaise closely with the planned pathway to hand-over care provided. Working on the unplanned pathway requires the post holder to be proactive in problem solving considering the patient holistically, including ordering of necessary equipment and prescribing and liaising with the multidisciplinary team.
