- To assess by telephone people's access to community support and identify needs to prevent readmission to hospital by enabling advice and guidance to signpost to relevant services.
- Helping people to find and use local services that they need and if necessary, refer and follow up on referral outcomes.
- To provide a holistic approach to triage and open conversations around needs identified upon hospital discharges.
- Provide evidence-based information to people about admission avoidance related to access to community support, self-management, and general wellbeing.
