Job summary
Work within a community nursing team visiting patients in there home including care settings to deliver delegated nursing care.
We are looking for someone who fulfils the requirements on either job description. Both are attached.
Main duties of the job
To assist the registered staff in providing a highly specialized Community Nursing Service to adults. This involves assisting the registered staff inensuring appropriate pathways of care for patients who require those services.To contribute to the assessment and the treatment of individual patients on the caseload and communicate any changes in care needs to theregistered staff.To communicate with patients, relatives, carers and health care professionals to assist the registered staff in ensuring an integrated, holisticapproach to patient care.
