The Paediatric Bladder and Bowel Team are recruiting to a specialist support worker post working part-time 24 hours a week. The team are a flexible team working Monday to Friday based primarily at Sherwood Rise Health Centre and undertake clinics both at other health centre bases and specialist schools.

We are looking for an enthusiastic, motivated, and conscientious support worker with an interest in paediatric continence to join the team within the Bladder and Bowel Service. We are a dynamic, developing service providing education, advice, support, and clinical care.

You will be supporting the Paediatric Specialist Bladder and Bowel Nurses with children and young people some of which will have disabilities and complex needs. The role will involve continence assessments, treatment plan reviews, education, product reviews and continence promotion with the support of the specialist nursing team.

You will need to demonstrate excellent interpersonal and communication skills. You must be able to manage tasks efficiently and effectively manage your time. Ability to work flexibly within the team is advantageous.

The hours of work will be 8.30 5.00 excluding weekends and bank holidays.

Main duties of the job

- Act with the Paediatric Bladder and Bowel Specialist Nurses as a key worker for children with complex continence needs within the family and in specialist schools.

- As a health professional, become involved with co-ordinating pathways of care and help develop the service to meet national guidance and standards.

- Provide assessment, advice and support empowering families in supporting children to manage their incontinence and/or promote continence.

- Provide assessment and review for the appropriate use of continence products.

- Act as a liaison between hospital and home.

- Act as a specialist resource by providing education and advice to multi-disciplinary staff.

- Attend multi-agency professional meetings if required.

The post holder will work within the established Paediatric Bladder and Bowel Specialist Service which delivers a high standard of specialist childrens continence care to patients and their carers and those who work with children and young people with continence problems.

Through both a mixture of general and specific care and teaching; to meet the needs of identified children and their families, particularly in relation to promoting continence, toileting skills and continence product provision assessments.

The post holder provides education, advice and support to children, families and professionals to support and empower them to manage the childs continence.

They will work alongside the specialist nurses in this small team to promote the health and wellbeing of children and their families and ensure the provision of high quality, safe care to children and families on the caseload. They will work closely with other health professionals within the Trust and across the health economy, as well as with statutory and third sector agencies.

Undertake the on-going assessment of an identified caseload of children, some of whom will have behavioural issues and disabilities, requiring continence products and identify those who may benefit from toilet training.

Organise continence products/supplies and ensure families/carers are trained in their use, ensuring appropriate products are provided.

Manage a caseload of children, under the supervision of the Specialist Nurse, with continence needs, assessing, planning, implementing and evaluating their individual programme of care.

Liaise with Paediatricians and other multi-disciplinary/agency staff.

Plan, implement and monitor toilet training programmes working with children and their parents, provide ongoing support and feedback to the referrer as required.

Identify child/parent learning needs and modify approach accordingly, develop strategies and tools for learning.

To actively involve patients in decisions about their continence.

Undertake assessments of developmental skills around toilet training providing advice toparents / professionals re suitability to commence a programme.

Performing and accurately recording height, weight and urinary investigation.

Be competent in interpreting bowel, fluid intake and output charts.

Monitor and evaluate agreed individualised care plans with supervision from the specialist nurses.

In conjunction with Paediatric Specialist Clinical Nurse Specialist contribute to the maintenance of high standards of care by developing evidence based protocols and guidelines and ensure they are introduced into all care settings, including homes, schools and other community settings.

Assist in problem solving to prevent continence/health deterioration and reduce hospital admissions.

Update specialist continence knowledge to ensure that your area of speciality is evidence based and in line with NICE/Trust guidelines.

Provide health education/promotion in a sensitive and tactful manner.

Within continence role attend clinics, seminars and relevant knowledge updates.

Report any untoward incidents in accordance with Trust Policy.

Keep accurate records and statistics.

Take part in any research or audit projects as required.

Assist in any tasks as required by the specialist nurses, appropriate to their level of competence.

Attend mandatory training.

Participate in the clinical supervision and appraisal process and take responsibility for reaching agreed developmental objectives.

Communicate information effectively with members of the multi-professional team both within the organisation and with external stakeholders and partners, to ensure care is delivered to children and young people both effectively and efficiently.

Participate in relevant patient related school and multi- agency meetings and Case Conferences as appropriate.

Ensure high standards of written and verbal advice are available to the children/young people and their families / carers using information agreed with the specialist nurse eg information leaflets, Care Plan.

Report and escalate as necessary any clinical care, safeguarding or patient safety concerns for the children/young people on the caseload.

Attend and contribute to multi-disciplinary team meetings, read emails and professional literature as required to pass on relevant information to both the CYPCT and wider CCNT.
