Job summary

Nottingham CityCare are excited to announce that we are recruiting to a Children and Young Peoples Practitioner (CYPP) post within the Southglade/Bestwood team.

Nottingham CityCare is a dynamic organisation, commissioned to provide NHS services in the community.

You will be part of our 0-19 Public Health team, working in various community settings and clients homes, supporting Health Visitors in the delivery of the 0-19 Healthy Child Programme, via our universal and targeted approaches. We are currently recruiting a CYPP that is committed to supporting and delivering public health messages to the families within Nottingham City. You will need to be a confident practitioner, who has excellent communication skills and a passion to embrace and support equality and diversity.

To support in post, you will have access to a plethora of mandatory and role specific training, with protected time for ongoing professional development. We have a robust competency package which will ensure you feel confident and competent to practice without direct supervision.

We also have a dedicated Safeguarding Service who will provide you with safeguarding advice, support and supervision. Our supervision package also includes management supervision and clinical/restorative supervision when required.

Main duties of the job

Your work will centre largely around supporting families who have been assessed to require some additional support in a number of areas, these are commonly referred to as Programmes of Care. This may be in relation to sleep, speech, language and communication, behaviour and parenting, continence and healthy lifestyles. On some occasions you may need to holistically assess a childs development and escalate any significant concerns to the caseload holder.

Most importantly, the successful candidate will need to be able to use verbal and non-verbal communication skill in order to engage families in meaningful conversations in keeping with evidence based key public health messages. They will need to have an approach which is restorative and empathetic. The post holder will be expected to have some knowledge of the specific needs unique to families residing within Nottingham City

Job Purpose

To work as part of an integrated Childrens Public Health 0 -19 Nursing team and support in the delivery of the Healthy Child Programme to children aged 0 to 19 years old in a variety of community settings.

Will participate in the development of a range of health promotion initiatives which aim towards improving the physical, social, and emotional well-being of children, young people and families.

Dimensions

The post holder will work as part of a geographical, skill mix team to support the delivery of high quality public health services to children, young people, and their families in Nottingham city.

Organise and complete delegated work effectively.

Undertake other duties as may be required for maintenance of an effective and efficient service.

Key Responsibilities

Quality and Performance

Undertake Healthy Child Programme, developmental reviews and public health interventions delegated by the Specialist Community Public Health Nurse.

Undertake specific public health programmes of care, such as behaviour/parenting support, sleep management support, continence, healthy lifestyle and speech and language assessments in a variety of settings e.g. home, clinics, childrens centres, and schools.

Be able to facilitate small groups to deliver health promotion messages e.g.weaning, oral health.

Providing routine information to children, young people and families which requires persuasive skills, particularly when there are barriers to understanding.

Receive information from clients who may have difficulty conveying their views and ensure provision is made to facilitate their ability to achieve this e.g.when English is not the first language, SEND.

To be able to assess the needs of families under the direction of a Specialist Community Public Health Nurse or Community Public Health Nurse using the relevant assessment tools

To use evidence-based programmes of care in line with CityCare policies and guidelines to deliver safe and effective care.

Undertake targeted health promotion initiatives in a variety of settings e.g. homes, clinics, schools.

Undertake any other duties which may be reasonably required within the remit of the role.

Ability to contribute towards achieving key performance targets.

To contribute to the writing of reports, feedback to the caseload holder and attend meetings/ reviews under supervision to support targeted families.

Maintain an accurate diary in accordance with the lone working policy and adhere to agile working guidance

Work in a cost-effective manner.

Contribute to processes to maintain stock control and ordering of resources required to undertake role.

Actively contribute to maintenance and security of equipment used within the team.

Contribute to activities aimed at increasing service efficiencies e.g., reduced printing and signposting to alternative resources.
