Job summary

An exciting opportunity has arisen for a full time Band 4 Respiratory Assistant Practitioner to join Nottingham CityCares Integrated Respiratory and Home Oxygen Service.

The Integrated Respiratory Service are a proactive, forward thinking and dynamic team which provides evidence based assessment and follow up for individuals requiring urgent and routine respiratory management in the community. The team provide an acute response service alongside chronic respiratory condition management across health and social care sectors. The team are based at Aspect house but work across a variety of settings in Nottingham city.

We are looking for an enthusiastic, motivated and conscientious team member who is willing to learn and work within the established Integrated Respiratory and Home Oxygen Service. You will deliver Pulmonary Rehabilitation sessions, manage a clinical caseload and complete oxygen assessments based within competencies set out within the service. Within the wider service you will be involved in development and support to other health care professionals.

You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritize tasks and manage time effectively, also be able to work in an integrated manner and flexibly within the team and be confident in completing individual tasks. You will need to hold a valid driving licence and have access to a vehicle for this role.

The hours of work will be between 8am and 5pm Monday to Friday.

Main duties of the job

To work alongside Nurses, Physiotherapists, community support workers and administrators to provide assessment and treatment intervention to patients with a confirmed respiratory diagnosis or requiring home oxygen.

To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values.

Job responsibilities

Job Purpose

To work alongside Nurses, Physiotherapists, community support workers and administrators to provide assessment and treatment intervention to patients with a confirmed respiratory diagnosis or requiring home oxygen.

To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values.

To maintain records as an autonomous practitioner working with supervision

Dimensions

You will be working in a variety of community settings including health centres, community centres, a patients own home or residential home following treatment plans and monitoring the patients condition/progress

Undertake clinical observations such as BP, temp, oxygen saturations capillary blood gases etc

Delivering Pulmonary Rehabilitation sessions

Managing a caseload of patients with a confirmed respiratory diagnosis.

You will participate actively in the services training strategy, including in-service training and multi-agency training.

Working in conjunction with and under supervision of senior clinical professionals.

Able to fulfil physical demands of role, including manual handling applications.

Key Responsibilities

Clinical

1. To undertake individual treatment and interventions with clients under the supervision of qualified staff.

2. To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner (AP) role.

3. To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user.

4. To promote wellbeing, for service users through Pulmonary Rehabilitation.

5. To attend and contribute to team meetings and case discussions using effective communication.

6. To maintain comprehensive and accurate records in line with employers polices.

7. To promote health and wellbeing with all citizens.

8. To undertake a range of MDT competencies, as required by the service.

9. To operate under the guidance of clinicians and only within the scope of AP practice.

Managerial and Leadership.

10. To provide support to other Community Support workers as appropriate.

Education and Development

11. To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines).

12. To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also to keep an active up to date CPD portfolio.

13. To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations.

14. To have regard at all times for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties.
