Job summary
An exciting opportunity has arisen for an enthusiastic Assistant Practitioner to join the Adult Bladder and Bowel Specialist Service. The Adult Bladder and Bowel team are part of Nottingham CityCare Partnerships Adult Services, based at Sherwood Rise Health Centre to provide specialist continence related care to housebound clients within Nottingham City.
Incontinence is a pervasive health issue and its impact on the quality of life for clients, their families and carers are widely acknowledged. However, the fact that bladder and bowel symptoms can be an indicator of more serious underlying health conditions, is less recognised meaning every person who experiences bladder and bowel symptoms should have their condition holistically assessed to understand aetiology.
The successful candidate will complete holistic continence assessments, translate outcomes into individualised management and treatment plans, implement initial care required and evaluate the effectiveness.
You must have a driving license and access to your own transport.
The core hours of the service are 08.30 16.30hrs Monday to Friday.

Main duties of the job
- Support the completion of holistic continence assessments and reassessments, delivery of individualised management and treatment plans as delegated by the Community Continence Registered Nurse, and support evaluation of the effectiveness for adult clients within a community setting,
- To support the provision of continence advice to clients, carers and those who work with clients with continence problems as advised by the Community Continence Registered Nurse,
- Maintain contemporaneous records as an autonomous practitioner working with supervision.
- To escalate clients to the Community Nurse Continence Care when their needs are beyond the Assistant Practitioner (AP) role.
- To accept responsibility for designated caseload, working independently taking an active role in promoting independence.
- To promote health and wellbeing, safe mobility, transfers and effective communication. To provide instruction, advice and intervention to clients and carers in line with the care plans, policies and Standard Operating Procedures.
- To attend and contribute to team meetings and case discussions using effective communication.
- To maintain comprehensive and accurate records in line with employers polices.
- To undertake a range of nursing competencies, as per the AP competency framework.
- To operate under the guidance of clinicians and only within the scope of AP practice.
