Job summary

Our health reablement and urgent care team are seeking caring, enthusiastic and dedicated Rehabilitation Support Workers with a sound knowledge and experience of working with complex, frail and older people.

Citycares Health Reablement and urgent care services deal with a wide range of cases, providing care and rehabilitation for people presenting with complex pathologies, as well as challenging medical and social circumstances. We are seeking staff with a wide range of holistic health and social care skills and experiences to compliment our service.

Visiting patients within their own homes, the teams are dedicated to preventing unnecessary hospital admissions and promoting independence and quality of life. You will be working closely with Occupational therapists, Physiotherapists and Nurses as well as GPs, District Nurses, Community Matrons, the acute trust and a range of Health and Social Care Professionals.

Main duties of the job

The aim of our service is to prevent unnecessary hospital admissions and to facilitate timely and safe discharges from the acute setting. We promote independence and with intensive rehabilitation and assessment, we strive to help our service users to achieve their goals, however big or small. Rehabilitation Support Workers practise under the guidance and supervision of the Clinical team.

To apply for this job you must have a full driving license and access to a car for work purposes. The hours of work will be between 7am - 10pm on a fixed rolling Rota basis (*out of hours operations* carries an enhancement as per AFC). The Service operates 7 days per week, 365 days per year including bank holidays.

To work with the at home teams to implement rehabilitation and ongoing care programmes to people referred to the urgent care and Reablement service.

To carry out goal focused programmes designed by the clinical team with the aim of maintaining people at home or returning people to their own home after a period of hospitalisation, illness or not coping.

To provide any essential care to maintain a persons health and wellbeing and feedback to the team regarding ongoing care needs.

The post holder will receive regular supervision from an appropriate member of the team.

Dimensions

This post has no budgetary responsibility; however the post holder is responsible for demonstrating cost consciousness.

You will be required to work across different bases and community settings which may include clients own home, residential or nursing homes or hospital

Work with a variable caseload allocated on a daily basis.

You will be required to complete all essential training in accordance with

CityCare policy and procedures plus any other identified training.

Key Responsibilities

Clinical

To undertake rehabilitation and evaluation of clients care programmes working either:

    • Alongside a clinician.
    • Independently in consultation with a senior member of the team.
    • To use skills in motivation, negotiation and facilitation to achieve desired outcomes.
    • To occasionally provide and fit equipment following an emergency or crisis intervention and assessment from a qualified member of staff.
    • To educate clients and their carers, under the guidance of a qualified member of staff in the safe and correct use of equipment.
    • To work in a variety of settings that may include clients own homes and care homes.
    • To ensure effective communication and exchange of information with clients/carers through direct contact, written communication and telephone contact.
    • To liaise with multi-professional teams and other agencies to ensure good outcomes for clients.
    • To adapt the approach used to take account of clients/carers individual circumstances and to feedback to relevant clinician. These may involve complex, unpredictable and difficult situations e.g. challenging behaviours, family dynamics or mental health needs.
    • To feedback, monitor and update on the progress and needs of clients/carers to the relevant clinical staff.
    • To write and maintain accurate and comprehensive records in line with

City Care Policy and procedures.

Organisational

To comply with health and safety requirements and to maintain and adhere to the organisations policies and procedures.

To undertake administrative, clerical and housekeeping duties to promote good practice and organisation within the team.

To ensure effective use of time and organisational skills to maintain a range of activities which may require flexibility at short notice.

To maintain appropriate statistical returns in line with CityCare and Legal requirements. This will involve data entry and storage in paper and/or computer format.

To assist in the organisation of home assessment visits and accompany the clinician when necessary.

To actively contribute to the promotion and marketing of the service.

To plan and prioritise own workload.

To assist with induction of new staff including taking basic training sessions

Education and Development

To attend organised training sessions and complete all relevant competences within agreed time frames.

To be involved in appraisal systems, clinical supervision, and be active in identifying aims and objectives for personal development in line with City care Policy.

To keep an up to date CPD portfolio.

To assist in the training and development of others (new members of staff, new RSWs, students and visitors to the service).

To keep up to date in basic IT skills in line with the data collection records.

To participate in appropriate in service training and individual learning with specific and measurable outcomes.

To undertake any other duties that may be reasonably required by clinicians.
