Job summary

Nottingham CityCare are excited to be adding an Advanced Diabetes Specialist Dietitian to our established and forward thinking integrated Diabetes team. We are seeking an individual who is motivated, a team player and can relate with CityCares values and behaviours to form part of our high performing, highly skilled team.

The successful candidate will play a crucial clinical role in the person-centred assessment, planning, and coordination of care and support for people in Nottingham City with diabetes.

The aim of this role is to promote engagement with appropriate services, contribute to more effective specialist management, and prevent potential negative health outcomes of diabetes, including avoiding unnecessary diabetes-related hospital admissions.

This multi-disciplinary team works in partnership with Nottingham University Hospitals trust and wider community services and consists of Consultants, Diabetes Specialist Nurses, Dietitians, Educators, Support Workers and Administrators delivers high quality community-based Diabetes care to the population of Nottingham City.

As part of this recruitment process the service will consider a B6 to B7 development opportunity in line with CityCare's progressive culture and commitment to our strategic aim to provide rewarding working lives and careers.

This post is working part time 15 hours per week.

Main duties of the job

We are looking for an experienced dietitian to lead, manage and collaborate within the integrated diabetes team to ensure effective care for the people within Nottingham City who live with Type 1 & 2 Diabetes and related co-morbidities.

The post holder will be expected to support the development, planning and implementation of local service strategies whilst ensuring compliance to both local and national guidelines.

The Dietitian will need to demonstrate evidence expertise within the long-term health condition, Diabetes using evidenced based practice to develop education and training to both people living with diabetes and other healthcare professionals.

There is a clinical patient facing element to this role which includes the delivery of multi-disciplinary clinics,domiciliary visits and structured education. There will also be opportunity to support the development of a community insulin pump service working in collaboration with Nottingham University Hospitals.

This position includes the direct line management and support of other dietitians proportionate to the position hours.

Key Responsibilities

Clinical

  • Receiving, interpreting and prioritising relevant referrals from a variety of health and social service professionals for patients with diabetes
  • To be an expert source of knowledge, therapeutic dietary advice and treatment recommendations for patients with diabetes and health professionals
  • To develop individual patient centred care plans based on nutritional requirements including interpretation of biochemistry, anthropometry, clinical conditions, medication, diet history, family and social history.
  • Provide appropriate dietary counselling, therapeutic, lifestyle advice to patients and carers. This will include domiciliary settings where appropriate
  • To undertake patient facing work within various venues across Nottingham city, including joint clinics with other members of the diabetes team e.g. Consultant and Diabetes Specialist Nurse, this may also include domiciliary visits
  • To deliver DAFNE structured education programme to the citizens of Nottingham City.
  • Attending clinical and multi-disciplinary team meetings as required.
  • Maintaining accurate and comprehensive patient records. Managing own clinical caseload
  • Undertaking data collection e.g. the use of SystmOne and other packages to collect patient data statistics and undertake audit.
