Job summary

This is your opportunity to join Nottingham CityCare's highly regarded MOSAIC (Musculoskeletal Orthopaedic Spinal, Pain, and Integrated Chronic fatigue syndrome) Service as an Advanced Practitioner Physiotherapist across the newly appointed Primary Care Networks (PCNs) in Nottingham City.

Employed as an Advanced Practitioner Physiotherapist, your First Contact Practitioner role, will require you to be assigned to a Primary Care Network where you will perform advanced MSK assessments, diagnosis and first line treatment and (if appropriate) investigations and referrals with the overarching aim to reduce primary care resources.

This post is ideal for experienced clinicians that can demonstrate a high degree of autonomy and complex decision making across a range of settings, enabling innovative solutions to enhance patients experience and improve outcomes. Successful candidates will have completed or started to gather evidence for Stage 1 & 2 of The First Contact Practitioner roadmap.

MOSAIC is a large, friendly and innovative team of physiotherapists, occupational therapists, pain consultants, specialist GPs and exercise rehabilitation practitioners.

The ability to travel between clinic locations is essential. Working hours are ideally between 07:45 and 6pm, but we are open to flexible working patterns for the right candidate.

Job Purpose

  • Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care (including in relation to undifferentiated conditions). This will involve seeing patients, without prior contact with their GP, in order to establish a rapid and accurate diagnosis and management plan.
  • Progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of particular tests diagnostic services such as x-rays and blood test, and interpret and act on results to aid diagnosis and the management plans of patients.
  • Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions.

Dimensions

  • To be responsible for the delivery of the MOSAIC service for clients referred by GPs, other health professionals, social services and self-referral.
  • To work autonomously as a First Contact Physiotherapist within Nottingham's Primary Care Networks.
  • To supervise less experienced physiotherapists, support staff, and students. To supervise, educate and assess the performance of under graduate physiotherapy students as required, ensuring to the standard of practice and teaching meets the standards set by the degree level qualification.
  • To work in an extended scope including but not exclusive to:

  • Low Back Pain triage of secondary care referrals
  • Clinical assessment and treatment planning in Integrated Clinical Assessment and Treatment (ICATS) clinics

  • Administering and evaluating outcome of injection therapy
  • To monitor and maintain performance of clinical sub-team of physiotherapists

Clinical

  • To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care and to support more junior staff to do likewise.
  • To have a lead role and determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each client.

  • To provide highly specialised advice, teaching and training to other members of the primary health care team regarding physiotherapy management of clients with problems related to rehabilitation, ageing and obesity.
