Job summary

Are you a qualified Physiotherapist who wants to make a difference? Do you want to be involved in the continued development of an innovative integrated service? As an autonomous member of a well-supported therapy team within Nottingham CityCare Partnerships Integrated Care Homes Service, your aim will be to optimise clients independence and prevent hospital admissions.

As an NHS Health Professional, you will provide integrated holistic care to a wide range of clients, by in reaching into Nottingham City Care Homes to deliver NHS services. The services overarching aim is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. A single point of access enables the care home staff to refer patients directly to the service enabling the prioritisation and allocation of referrals to the most appropriate professional to meet their need.

As this role is based across Nottingham City, you will be required to hold a UK driving licence and have access to a vehicle for work purposes.

Main duties of the job

This Physiotherapy role is part of the therapy element of the Integrated Care Home Service. This element comprises of both physical health and dementia specialist therapists and Assistant Practitioners, working as an integrated therapy team, supported by a therapy lead.

Your caseload will comprise of adults living in care homes who have either complex physical health needs or are living with dementia. In addition to your proven physical therapy skills, you will have the opportunity to develop your knowledge and skills working with professionals from the specialist services, who in reach into care homes giving you invaluable CPD to support your continued learning and development.

All referrals to the therapy element of the service are triaged by one of the therapy team and assigned to the most appropriate therapist for the patients needs.

They will provide supervision to the Assistant Practitioners within the team, and act as a resource to other members of the service.

As an Integrated Service there is an emphasis on collaborative working across all the elements.

There are wider therapy meetings, led by the clinical lead, with therapists from other CityCare services who in reach into the care homes. These meetings enable a sharing of knowledge and the development of new and innovative ways of working. The Physiotherapist will be involved in contributing to the evaluation and audits of service provision.

Job Purpose

  • To assess, plan and develop treatment programmes for Nottingham City patients within care homes who have a wide variety of complex physical needs, to maximise the physical and social environments to help people engage in activities of daily living and contribute to multi-disciplinary treatment planning.
  • Determine clinical diagnosis and physiotherapy treatment indicated and to maintain records as an autonomous practitioner.
  • To be responsible for continuing the ongoing development of high standards of clinical practice and expertise within the service contributing to implementation of specific changes to practice and service protocols and making a professional contribution to the work of a multi-disciplinary team.
  • To advise and support care home staff in maintaining a residents wellbeing and engagement despite.
  • To manage/care coordinate a caseload of patients, and will provide advice, education and teaching sessions to providers of residential/nursing care.

Key Responsibilities

Clinical

  1. To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. To ensure a high standard of clinical care for the clients under physiotherapy management, and support more junior staff to do likewise.
  2. To determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each client and/or carer. This may include dealing with complex and conflicting opinions within the multi-disciplinary multi-agency team.
  3. To undertake the comprehensive assessment of clients, including those with a complex and/or chronic presentation using investigative and analytical skills through standardised and non-standardised assessments. To formulate individualised management and treatment plans, using clinical reasoning and using a wide range of treatment skills and options to devise a specialised programme of care.
  4. To set physiotherapy rehabilitation goals, carry out treatment interventions.
  5. To accept clinical responsibility for a designated caseload of clients and to organise this effectively with regard to clinical priorities and use of time.
  6. To provide highly specialised advice, teaching and training to other members of the Integrated Team regarding the physiotherapy management of clients with problems related to rehabilitation/mobility/ageing.
  7. To train, supervise and manage more junior staff, support staff and students. This will include the use of formal appraisal documentation and accreditation as a Physiotherapy Clinical Educator. To supervise, educate and assess the performance of undergraduate Physiotherapy students as required to ensure the standard of practice and teaching meets the standards set by the degree level qualification
  8. To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment.
  9. To work as a lone practitioner with supervision from band 7 Physiotherapist, the Team Leader will also provide support as required.
  10. To provide spontaneous and planned advice, teaching and instruction to clients, carers and other professionals to promote understanding of the aims of physiotherapy and to ensure a consistent approach to client care.
  11. To assess advice and, where appropriate, provide mobility aids including teaching clients/carers in their use.

1. To participate in the development of good practice for Physiotherapists within Nottingham CityCare Partnership CIC under the guidance of the Professional Lead Physiotherapist

2. To demonstrate a commitment to evidence-based practice.

3. To be responsible for own Continuing Professional Development under the supervision of the Band 7 Clinical Therapy Lead, keeping abreast of new trends and developments, incorporating them into your work as necessary. Attend relevant conferences and internal/external training courses. Support more junior staff to do likewise.

4. To maintain a high level of physiotherapy standards and work to the Physiotherapy Code of Professional Conduct.

5. To actively participate in the services training strategy, including in-service training, Trust and multi-agency training. To participate by attendance and presentation at staff meetings, tutorials and training sessions.
