Job summary
An opportunity has arisen for an enthusiastic, conscientious, passionate and motivated occupational therapist with a passion for working in the community and a keen interest in the holistic working model, to join the Integrated Reablement Team, incorporating the Urgent Community Response Service, operating across Nottingham City.
The service consists of a multi-disciplinary team (MDT) that includes Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Registered Medicine Technician, Assistant Practitioners, Rehabilitation Support Workers and administrative support. The aim of the MDT is to promote, improve and maintain independence through rehabilitation and crisis management with citizens in their own home or within a short term care setting to prevent unnecessary hospital admissions and support hospital discharges.
You should have an understanding of Health and Social care provision and the ability to work closely and collaboratively with partner organisations and other stakeholders. We respond to urgent referrals from a variety of sources such as Ambulance services, GPs, Social Services etc.
You will be supported to hone your assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and reablement programmes, as well as complex discharge planning experience.
Main duties of the job
- To work as an autonomous practitioner and undertake comprehensive assessments to identify the appropriate service for the clients referred.
- To carry out profession specific, and holistic, assessments and evaluation as required.
- To assist the multi-disciplinary team to plan and develop a goal focused rehabilitation/reablement programme for adults with a wide variety of complex physical and mental health needs either in a community bed or in their own homes.
- To be responsible for continuing the ongoing development of high standards of clinical practice with evidence based focus.
- To work as required within the Urgent Community Response and Integrated reablement team, responding to health and social care referrals with the aim of preventing admission to hospital or a care home, facilitating discharges from community beds and acute settings and promote independence.
- Able to fulfil physical demands of role, including manual handling applications.
- Will be required to travel city-wide for client visits, meetings and training.
- To participate in rehabilitation treatment programmes with the MDT
- To carry out training of other members of staff.
- Support senior colleagues with the facilitation of student placements.
