Job summary
Fantastic opportunity for an enthusiastic and highly
motivated Band 5 physiotherapist to join the community rotational programme
within Nottingham CityCare Partnerships Adult Services. This distinctive programme will
support you with a bespoke education and training, regular supervision, and
CPD opportunities across the organisation. Surrounded by the best clinical
and support staff, you will be creating forward thinking and innovative
approaches to managing people with multiple pathology and complex health
needs. CityCare has a robust 12 month Preceptorship Programme,
and additional clinical supervision, support and training will be provided by
senior physiotherapists within the teams and the Professional Lead for Allied
Health Professions.
Six month rotations are with the MSK Clinics Team,
Integrated Respiratory Service, Urgent Care and Reablement Teams, including
our community beds and at home service. It is a great opportunity to utilise a comprehensive
and patient centred approach to management of neuromusculoskeletal,
respiratory and long term conditions, maximising function and independence,
admission avoidance and supported transfer or discharge.
For the post you will need to demonstrate initiative and excellent interpersonal and communication skills.
Main duties of the job
The post holder will provide evidence-based physiotherapy interventions, individual programmes and group sessions to clients with a variety of health conditions and rehabilitation needs. This may be delivered in clinic/healthcare centre/community centre settings or in clients homes.
You must be able to prioritise tasks and manage time effectively, be able to work flexibly within a team and demonstrate understanding of working in a community setting.
As physiotherapist you will be expected to contribute to
high quality services by carrying out assessments, goal setting, implementing
treatment programmes and evaluation of clients who many have complex and/or
chronic presentations, including musculoskeletal, sensory impairments, medical
and/or mental health needs, with support and supervision from senior
physiotherapists.
You will be expected to work with the MOSAIC
(Musculoskeletal), integrated respiratory, health reablement (community beds
and at home service). This may extent or change to other community services
in-line with organisational development.
A large part of the role will be to support
patients/clients to improve or maintain their level of function and quality of
life and plan future care in an appropriate setting, along with organising
discharge planning in partnership with multi-disciplinary team colleagues, along
with outside partner organisations such as adult social care and other
voluntary services.

- To advocate on behalf of clients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the client.
- To be involved in the development of good practice for physiotherapists within Nottingham CityCare Partnership CIC and Social care under the guidance of senior staff.
