Job responsibilities

Job Purpose

To contribute to a high quality service by carrying out assessment, goal setting, implementing treatment programmes and evaluation of adult clients who may have complex and/ or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from senior physiotherapists.

To work within Community Beds Team within Health Reablement service . This may extend or change to other community services in-line with organisational development.

To support clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting and referring on to health services, voluntary services and public sector agencies.

To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training.

To be responsible for planning a safe discharge from the service.

To act as Key Worker as required.

Dimensions

Responsible for Occupational Therapy assessment and treatment for a defined clinical caseload appropriate to the service area.

Working in conjunction with and under supervision of a senior OT and other senior clinical professionals.

Lead group sessions where appropriate.

Able to fulfil physical demands of role, including manual handling applications.

Will be required to travel city-wide for client visits, meetings and training.

To participate in rehabilitation treatment programmes with the MDT.

To carry out training of other members of staff.

Support senior colleagues with the facilitation of student placements.

Clinical Skills

To be professionally and legally responsible and accountable for all aspects of your own work including the management of clients in your care.

To ensure a high standard of clinical care for the clients under your management, and support other team members to do likewise, implementing appropriate teaching, supervision and delegation skills where appropriate.

To assess for, provide, demonstrate and fit adaptive equipment using problem solving, and analytical skills.

To accept clinical responsibility for a designated caseload of clients, and to organise this effectively with regard to clinical priorities and use of time and ability to be flexible and respond at short notice to the needs of the service.

Working within a legal framework, gain informed consent to treatment and have the ability to identify through assessment, clients who lack the capacity to consent.

To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to a work situation.

To conduct and report on clinical risk assessments as and when appropriate.

Professional

To undertake the comprehensive assessment of clients, including those with a complex presentation using investigative and analytical skills through standardised and non-standardised assessments. Formulate individualised treatment plans with rehabilitation goals using clinical reasoning in consultation with senior therapists as required. This should be based on sound theoretical knowledge, utilising a wide range of treatment skills, and options to formulate a specialised programme of care.

To advocate on behalf of clients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the client.

To be involved in the development of good practice for physiotherapists within Nottingham CityCare Partnership CIC and Social care under the guidance of senior staff.

To complete patient/client records in line with organisation, regulatory body and legal requirements.

To represent the service externally and actively participate in the services marketing strategy when required, conducting presentations to a wider audience, including voluntary and public sector agencies.

To demonstrate a commitment to evidence-based practice and the use of outcome measures and make recommendations for change. To have a good understanding of the research process, and be willing to participate in local research and audit when appropriate.

To be responsible for own Continuing Professional Development, keeping abreast of new recommendations, research and developments, incorporating them into your work as necessary. Attend relevant conferences and external training courses. Support more junior staff to do likewise.

To maintain a high level of professional standards and work to the Professional Values and Behaviours as set by the OT professional body.

Policy

To be actively involved in Clinical Governance, implement policies and proposing practice changes.

To comply with organisations Lone Working Policy and local team guidelines at all times when working alone.

Planning and Organisation

To be responsible for own time and caseload management.

To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership CIC and Social Care.

To use information technology on a daily basis to record information and statistics, to aid communication and produce reports.

To ensure the Health and Safety of staff and clients at all times in accordance with Health and Safety legislation, Trust policy and procedures.

To undertake any other duties which may be reasonably required.

Training

To comply with and complete statutory and mandatory training in line with Nottingham CityCare Partnership policy.

To attend training events as appropriate to support your continuing professional development.

To maintain a continuing professional development portfolio identifying learning objectives.

To actively participate in your own supervision, annual performance review and personal development plan.

Qualifications

Essential

• BSc/Diploma in Occupational Therapy.
• HCPC Registration.

Desirable

• Relevant postgraduate education and training.
• Manual Handling.
• Safeguarding Children & Adults.
• Specialist interest in older people and dementia.

Experience

Essential

• Relevant undergraduate experience working with adults including Health Care of the Elderly and Older peoples mental health / Integrated Medicine, Orthopaedics, Neurology and Musculoskeletal.
• Understanding of multi-disciplinary and/or multi-agency working.
• Competence in individual caseload management and ability to manage time / work under pressure/in terms of risk and priorities.
• Customer service skills.

Desirable

• Post graduate experience working with adults including orthopaedics, neurology, falls prevention and musculoskeletal.
• Full KSF for the role/other relevant competency framework.
• Community experience. Lone working.
• Experience of working within multicultural communities.
• Understanding of Social Enterprise.

Special Requirements

Essential

• All appointments are subject to satisfactory medical clearance and the ability to cope with the demands of the role.
• Ability to undertake frequent moderate physical effort for several short periods during the working day. Activities may include manoeuvring people, and operating equipment. Periods of prolonged concentration as work pattern dictates and some exposure to distressing or emotional circumstances and occasional unpleasant conditions.
• Able to work unsocial hours and weekends within service hours of either 8:00-16:00, 12:00-20:00 Monday to Friday, 10:00-18;00 at the weekend.
• Full driving licence, use of vehicle for work with business insurance.
