Job summary
An exciting opportunity has arisen for an enthusiastic, conscientious, passionate and motivated occupational therapist with a passion for working with our community beds team. We are the clinical beds team with Nottingham city, that provide assessment and rehabilitation to patients who are unable to be discharged home from hospital due to requiring a period of assessment or rehabilitation, or to patients who require a step up from the community to prevent a hospital admission.
The service consists of a multi-disciplinary team (MDT) that includes Physiotherapists, Occupational Therapists, Nurses, Registered Mental Health Nurses, Assistant Practitioners, Rehabilitation Support Workers, and administrative support. The aim of the MDT is to provide holistic assessments to patients within our clinical bed, promoting independence and arranging safe discharges either back to the patients own home or on to long term care.
You should have an understanding of Health and Social care provision and the ability to work closely and collaboratively with partner organisations and other stakeholders, such as social care and our community health partners.
You will be supported to hone your assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and reablement programmes, as well as complex discharge planning experience.
CityCare will offer you an excellent Preceptorship Programme to support and develop newly registered AHPs.
Main duties of the job
To work as an autonomous practitioner and undertake comprehensive assessments to identify the appropriate goals to promote a return to independence.
To carry out profession specific, and holistic, assessments and evaluation as required.
To assist the multi-disciplinary team to plan and develop a goal focused rehabilitation/reablement programme for adults with a wide variety of complex physical and mental health needs in a community bed.
To be responsible for continuing the ongoing development of high standards of clinical practice with evidence based focus.
Able to fulfil physical demands of role, including manual handling applications.
Will be required to travel city-wide for clinical practice with the clinical beds, meetings and training.
To participate in rehabilitation treatment programmes with the MDT.
To carry out training of other members of staff.
Support senior colleagues with the facilitation of student placements.
