Job summary

We are pleased to be able to offer this full-time Band 5 Physiotherapist role with the expectation of the successful candidate being able to progress to Band 6 upon completion of the Band 6 Senior Physiotherapist competencies. This may take 6 months to 2 years depending on your current experience and skills.

The Community Rehabilitation and Falls Service works with a diverse caseload of patients in need of rehabilitation following an acute illness or injury, as well as those with deteriorating conditions where the focus is maintaining function and independence. The service has a particular specialism in falls prevention and rehabilitation.

Working in this field is challenging in its diversity, but offers an opportunity to use and build skills in key areas such as Falls, Neuro, Frailty, and MSK. Supervision, support and training will be provided by Senior Physiotherapists within the team, as well as Senior Occupational Therapists, and our very experienced and capable Assistant Practitioners and an Administrator who will further support your development.

Main duties of the job

The role involves providing physiotherapy assessment and intervention for rehabilitation, promoting independence, and falls prevention. With a varied caseload, you will need to evidence at application and interview that you have at least undergraduate experience in Neurology, MSK/Orthopaedics, and Health Care of the Elderly. Postgraduate experience would be an advantage.

You will work alongside colleagues within a multidisciplinary team to deliver exercise programmes, gait rehabilitation, provision of mobility aids, and falls prevention strategies. As the assessment and treatment takes place in patients own homes, you will need to be a car driver in order to carry out the role.

Experience of working in the community and as part of a multi-disciplinary team will be advantageous, and may mean that you are able to progress to a Band 6 role more quickly than if you do not have this experience at the point of application.

To support clients to improve or maintain their level of function and quality of life, and in particular to reduce the risk of falls, in partnership with the multi-disciplinary team (MDT). This will include liaising with and referring on to other health services, voluntary services and public sector agencies, and planning safe and effective discharge from the service.

To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training.

Dimensions

Responsible for Physiotherapy assessment and treatment for a defined clinical caseload appropriate to the service area, working in conjunction with and under supervision of a Senior Physiotherapist and other senior clinical professionals.

To participate in rehabilitation treatment programmes with the MDT, including leading group rehabilitation classes as appropriate.

To contribute to the supervision and training of Assistant Practitioners and Rehabilitation Support Workers, and to support senior colleagues with delivering student placements

Able to fulfil physical demands of role, including manual handling applications.

Will be required to travel city-wide for patient visits, meetings and training.

The role has no specific budgetary responsibility, however there will be a requirement to demonstrate cost-consciousness within the role.

Clinical and professional

  • To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care.

  • To undertake the comprehensive assessment of patients, including those with a complex presentation with the support of senior physiotherapists. Formulate individualised treatment plans with rehabilitation goals using clinical reasoning in consultation with senior therapists as required. This should be based on sound theoretical knowledge, utilising a wide range of treatment skills, and options to formulate a specialised programme of care.
  • To advocate on behalf of patients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the patient.
  • To be involved in the development of good practice for physiotherapists within Nottingham CityCare Partnership CIC and Social care under the guidance of senior staff.
  • To complete patient records in line with organisation, regulatory body and legal requirements.
  • To demonstrate a commitment to evidence-based practice and the use of outcome measures and make recommendations for change. To have a good understanding of the research process, and be willing to participate in local research and audit when appropriate.
