Job summary

Nottingham CityCare are looking for a Community Matron to join our Neighbourhood Teams within Community Nursing within Nottingham City (working 22.5 hours per week over 3 days one of which has to include a Friday).

Our Community Matrons are an integral part of our community nursing teams providing clinical expertise and knowledge to our patients. Providing support to community services for management of complex conditions and supporting our communities to stay and be managed at home.

You will be part of the wider community nursing team, working alongside other specialists nurses, GPs , secondary care, social services and the voluntary sector.

Main duties of the job

As a community matron you are a point of contact for care, support or as advanced nurse practitioners providing a variety of tasks and responsibilities, including:

  • Carrying out physical examinations.
  • Deciding on and carrying out treatment, including the prescribing medicines, or refer patients to an appropriate specialist.
  • Using their expert knowledge and clinical judgment to identify the potential diagnosis and where appropriate make a final diagnosis.
  • Using their extensive practice experience to plan and provide skilled and competent care that meets patients' health and social care needs, involving other members of the healthcare team as appropriate.
  • Ensuring the provision of continuity of care, including follow-up visits.
  • Assessing and evaluating, with patients, the effectiveness of the treatment and care provided and make changes as needed.
  • Working independently, although often as part the neighbourhood / district nursing team.
  • Providing leadership.
  • Making sure that each patient's treatment and care is based on best practice.
  • Seeing patients in their own home.
  • Working as part of multi-professional and multi-agency teams.
  • Providing supervision, education and support to members of the community nursing team.

Independent prescriber desirable or willingness to complete the course.

Job Purpose

  • Have a clinical role, with responsibility for planning, managing and co-ordinating the care of people with highly complex needs and long term conditions within a defined caseload.
  • Use caseload management techniques to reduced unplanned hospital admissions caused by poor control of long term conditions.
  • Develop advanced skills and competencies in prescribing, health assessment, medicines management, leadership and case management.
  • Work collaboratively across Primary, Secondary, Social Care and the Voluntary sector to establish a whole systems approach to managing Health and Social care for the designated patient group.
  • Take a lead role in clinical practice and strategic development for Nursing and Primary Health Care teams.

  • Using expert knowledge, advanced nursing and autonomous decision making skills manage a caseload of patients in the defined client group.
  • Comprehensively assess, review and evaluate the needs of both patients and their carers to improve their physical and psychological wellbeing whilst reducing acute exacerbation of underlying conditions and need for hospitalisation.
  • Work in partnership with patients, carers, GPs, consultants, other health professionals and social care as appropriate to instigate diagnostic testing and therapeutic treatments, ascertain diagnosis, develop, initiate and implement proactive care plans.
  • To develop advanced specialist clinical skills to undertake interventions consistent with evidenced based practice in line with national and local policy.
  • Use knowledge of medicines and prescribing skills to influence the reduction of risk and complications associated with medication and polypharmacy. To work closely with prescribing advisors and pharmacies as appropriate. To be responsible for the promotion of best practice that ensures the health, safety and security of self, patients and colleagues in accordance with the policies procedures and guidance of the organisation.
