Job summary

An exciting opportunity for District Nurse Lead role within Nottingham CityCare influencing, supporting and addressing the needs of both the professional and the leadership agenda of community nursing services.

With increasing health inequalities and complexity of patient care these roles with further enhance the Community Nursing roles within the PCNs. With the opportunity to look innovatively at service delivery, training developing and support for the clinical staff.

Main duties of the job

This District Nurse Lead role will support and address the needs of both the professional and the leadership agenda of community services. The role is a specialist nurse has direct responsibility for a complex caseload whilst providing specialist training, advice, mentorship teaching and support to Nurses and their teams.

Leading on complex clinical caseload including long term conditions and multi pathologies. Providing specialist clinical advice to their teams using evidence based research and practice. Providing clinical supervision and mentorship to band 6 District Nurse, Senior Staff Nurses and Assistant Practitioners. Providing professional support to the management and leadership delivery of the service.

Job Purpose

This District Nurse Lead role will support and address the needs of both the professional and the leadership agenda of community services. The role is a specialist nurse has direct responsibility for a complex caseload whilst providing specialist training, advice, mentorship teaching and support to Nurses and their teams.

Key Responsibilities

Clinical Practice

  • Utilises highly developed specialised and advanced clinical knowledge covering a range of procedures and underpinned by relevant broad-based knowledge, experience, and competence as a qualified District Nurse
  • Where appropriate to service delivery, demonstrates highly developed physical skills requiring accuracy and dexterity e.g., moving and handling techniques, clinical procedures, diagnostic tests.
  • Demonstrates independent practice responding to social, scientific, clinical, and ethical issues which are encountered.
  • Makes complex judgements requiring analysis, interpretation and comparison of options by integrating complex knowledge sources in new and/or unfamiliar contexts.
  • Responsible for complex clinical pathways
  • Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of self-care and empowerment Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a highly specialist standard
  • Prioritises their own workload and that of the wider team (specifically that of the band 6 District Nurses) within agreed objectives deciding when to refer to others as appropriate.
