Dementia Specialist Occupational Therapist

Job summary

As a member of the multi-disciplinary Dementia Outreach Team within Nottingham CityCare Partnerships Integrated Care Homes Service (ICHS), you will carry out specialist assessments of care home residents living with dementia, develop treatment plans using your clinical knowledge and skills to maximise the physical and social environments to help people engage in activities of daily living (working 15 hrs, 2 days per week)

Applicants should show evidence of strong OT skills and experience dealing with those living with dementia, ideally within community environments. Experience around contracture management and caseload management is essential.

Professional development is actively supported, with opportunities to work alongside specialist services and participate in ongoing education. Team members benefit from regular therapy meetings, peer support, and supervision.

The Dementia Outreach and Therapy teams play a vital role in optimising care home residents wellbeing, bringing together interdisciplinary expertise focused on person-centred rehabilitation. The DOT element of the service is currently undergoing a ICS wide service review.

Main duties of the job

This OT role is part of the Dementia Outreach (DOT) element of the Integrated Care Home Service. This element comprises of dementia specialist therapists, RMNs and Assistant Practitioners. The DOT therapists work alongside the physical therapy clinicians within the service as an integrated therapy team, supported by a therapy lead.

The DOT OT is required to possess a high level of clinical knowledge and experience in dementia care. They are responsible for the holistic assessment of patients, prescribing individual programmes of therapy for the patient and will hold a clinical caseload.

They will provide supervision to Assistant Practitioners within the team, and act as a resource to other members of the service.

As an Integrated Service there is an emphasis on collaborative working across all the elements and with therapists from other services who in reach into care homes. Wider therapy meetings, led by the clinical lead, with therapists from other CityCare services take place enabling a sharing of knowledge and the development of new and innovative ways of working.

The service provides support to care home staff, enabling them to deliver optimal care to their residents. This includes delivering training and education sessions to staff within the care home setting to upskill and empower them to deliver optimal care to their residents. As well as developing joint handovers within care homes to enable proactive care and prevent avoidable hospital admissions.

  • To be part of a multi-disciplinary Integrated Care Homes Service, responsible for the delivery of an occupational therapy service to residents with mental health problems related to old age, particularly those with dementia.
  • To supervise less experienced staff and students working with the Post Holder. To supervise, educate and assess the performance of undergraduate Occupational Therapy students as required ensuring the standard of practice and teaching meets the standards set by the degree level qualification.
  • To increase the capacity, confidence, competence, and willingness of partner organisations to provide the right level of care to this vulnerable client group and by doing so increase standards of care, improve quality of life, increase independence and choice.

Key Responsibilities

Clinical

  • To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. To ensure a high standard of clinical care for the clients under occupational therapy management and support junior staff to do likewise.
  • To determine by assessment and liaison with other relevant agencies the advice, education, and treatment requirements of each patient. This may include dealing with complex and conflicting opinions within the multi-disciplinary multi-agency team.
  • To undertake the comprehensive assessment of clients, including those with a complex and/or chronic presentation using investigative and analytical skills through standardised and non-standardised assessments. To formulate individualised management and treatment plans, using clinical reasoning and using a wide range of treatment skills and options to devise a specialised programme of care.
  • To set occupational therapy goals and carry out treatment interventions.
  • To carry out manual handling assessments of clients living in care homes where there is a primary mental health concern.
  • To accept clinical responsibility for a designated caseload of clients and to organise this effectively with regard to clinical priorities and use of time.
  • To provide highly specialised advice, teaching and training to other members of the Integrated Team and other agencies regarding the occupational therapy management of clients with problems related to ageing and dementia.
  • To train, supervise and manage more junior staff, support staff and students. To supervise, educate and assess the performance of undergraduate occupational therapy students as required to ensure the standard of practice and teaching meets the standards set by the degree level qualification.
